Home Health Triage LPN Saturday and Sunday 7AM-5:30PM, on-site for training at VNA Care, 67 Millbrook Street, Worcester. After training the role will be remote.

Summary: Receives and processes all agency telephone calls during assigned shift, assesses callers' needs and takes action to address those needs.

Education: Graduate of a certified Licensed Practical Nursing program. Licensed to practice in MA.

Experience: Minimum 2 years of nursing experience; one year of community health experience preferred. Previous triage or intake experience preferred.

Schedule: Saturday/Sunday 7:00a -5:30p: holidays coverage included.

Holidays are New Years, Memorial Day, Juneteenth, Independance Day, Labor Day, Thanksgiving and Christmas Day.
