Responsibilities of a Licensed Practical Nurse include:

  • Performs prescribed individualized patient care to patients as delegated.
  • Participates in patient case conferences as appropriate.
  • Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.
  • Observes and reports to the RN the patient's physical and emotional needs and reactions to treatment and to medications.
  • Provides nursing care in a way that considers patient's privacy, comfort and safety.
  • Demonstrates the ability to instruct patients in the home and at community programs.
  • Performs procedures based on the Agency's policies and procedures.
  • Performs selected procedures based on the Agency’s competency testing.
  • Administers medication after successful completion of a medication competency.
  • Accepts verbal orders from physicians.
  • Assists the RN in planned patient teaching.
  • Documents patient information in a clear, legible and accurate manner according to policy.
  • Documentation is completed within established time frames to meet Federal and State requirements.

Qualifications of a Licensed Practical Nurse include:

  • Must possess an active LPN license in both VT and NH.
  • 1-2 years’ experience in home care preferred.
  • Ability to communicate clearly, verbally, and in writing.
  • Flexible in adapting to work schedule changes in order to meet patient’s 24 hour needs.
