We are seeking a highly motivated and experienced Licensed Clinical Social Worker (LCSW) to provide in-home counseling and remote counseling services to older adults in Suffolk County. The LCSW will be responsible for conducting assessments, developing treatment plans, and providing individual and/or family therapy to clients in their homes and/or remotely.

Key Responsibilities:

• Conduct comprehensive assessments of clients' social, emotional, and psychological needs.
• Develop individualized treatment plans and set goals in collaboration with clients and their families.
• Provide individual therapy services to individuals in their home and remotely.
• Monitor clients' progress and make adjustments to treatment plans as needed.
• When necessary provide clients with community linkage to help address other needs.

Qualifications:

• Must hold a valid LCSW license in the state of New York
• Must have experience working with older adults
• Strong clinical and diagnostic skills
• Excellent communication and interpersonal skills
• Must be able to work independently
