The Community Liaison plays a crucial role in driving care consultations and sales initiatives, as well as fostering strategic partnerships within the community for our non-medical home care service organization. Community Liaison Responsibilities:

· Conduct care consultations with potential clients to understand their needs and develop personalized care plans in collaboration with their families.

· Cultivate partnerships with referral sources through educational initiatives, presentations, and events, serving as a knowledgeable resource for clients and their support systems.

· Drive sales and marketing efforts to expand business, leveraging existing relationships and establishing new connections within the community.

· Provide support and guidance to current clients through quality assurance visits, ensuring the delivery of exceptional care, and offering case management services as necessary.

· Engage with caregivers to address their individual needs, reinforcing their importance as valued team members.

Community Liaison Requirements:

· Demonstrated experience in building and nurturing positive relationships to drive business growth.

· Previous case management experience is strongly desired.

· Healthcare background or familiarity is preferred.
