Client Care Coordinator - Leading Home Care Agency

POSITION PURPOSE: The Client Care Coordinator is responsible for the coordination of services including, but not limited, to: client assessments, client follow up, care plan updates, starts of care, caregiver scheduling coordination and client challenge resolution.

Qualifications:

1. At least one year of care coordinator experience in the homecare or healthcare industry is required.

2. LPN certification desired, but not required.

3. Ability to learn and adapt in a fast-paced and ever-changing environment.

4. Strong interpersonal and communication skills, both verbal and written.

5. Proficiency with Google Drive applications and healthcare-related software.

6. Ability to remain calm, flexible, and resilient in challenging situations.

7. Excellent organizational and time management skills.

8. Present a well-groomed image that reflects the professionalism of the business.

9. Combination of sitting, standing, bending, reaching, stretching, stooping,

walking, climbing stairs and moving intermittently during working hours.

10.Successful completion and ongoing compliance with all licensing, certification, continuous education, background testing, drug and alcohol testing, OIG and health assessments as may be required under federal, state, or local law or per employer policy.

11.Must be legally authorized to work in the U.S.

The duties and responsibilities described below do not represent a comprehensive list for the position. Additional tasks may be assigned periodically as necessitated by business demands.

Responsibilities:

1. Conduct initial assessments and collect client documentation for start of care.

2. Update databases and software with client information and follow-up details.

3. Perform regular check-in calls with new and existing clients to ensure

satisfaction and address any concerns.

4. Coordinate client appointments including scheduling caregivers.

5. Identify and resolve client issues in a timely and professional manner.

6. Collaborate with the care team to update care plans and address client needs.

7. Maintain accurate documentation and records of client interactions and care plans.

8. Provide exceptional customer service to clients, their families, and referral

sources.

9. Adhere to company policies, procedures, and standards of care.

10.Dependable - arrive on time and complete assigned tasks with minimal

supervision.

11.Communicate and refer appropriate matters to the director for additional

guidance.

12.Maintain absolute confidentiality of all information pertaining to employees, clients and client’s families.

13.Work independently with minimum direction and/or supervision after training.

Classification: Exempt - Salary - Full time
