Visiting Angels is looking for a Client Care Coordinator to join our team at 7110 W. Central Ave. Suite A, Toledo, Ohio 43617 . The candidate will not be providing patient nursing care. Must be an LPN or RN with at least 2 years’ experience that includes some home care. Computer skills and experience with Microsoft Office required and must have the ability to readily adapt to other software programs. The position requires someone that is self-motivated, can work independently, has excellent customer service skills, can pass a background check, has a good driving record, can pass random drug screenings, and is able to perform all ADL services. The position requires in-home client visits to create or update Care Plans for their home care needs. Caregiver training and skills testing will also be required. Must be physically able to stoop, bend, crouch and lift 50lbs, and sit or stand for long periods. Primary work schedule is Monday through Friday, 8:00am to 4:00pm with travel in the Toledo Metropolitan area using company car during working hours. Compensation is for starting pay that will grow with company growth.

Duties may include, but are not limited to:

Onboarding, Home Assessments and Contracts for new clients

Client Care plans - creating and updating

Care Supervisory Visits

Caregiver training and testing

Communication with various services and agencies, as needed.

Working with Microsoft Office programs - creating and working with reports

Working with additional software programs

Requirements:

-Must be licensed in Ohio as an LPN or RN

-Must be 18 years of age or older

-Must have a valid driver's license, proof of auto insurance, and a reliable car.

-Must be able to pass a drug screen at any time

-Must have clear background check and driver's record

-Must have office/Microsoft experience

-Must have professional attire and demeanor

-Must provide professional resume

Job Type: Full-time

Pay: $45,000.00 - $55,000.00 per year
