Vision of Hope is Hiring a Referral coordinator Near Springfield, IL

SUMMARY

This is a temporary position, active from July 2024 through November 17th, 2024.

The Referral Coordinator plays a pivotal role in facilitating the coordination of client referrals within The Autism Clinics at Hope.

Responsibilities include acting as a liaison between patients and the clinical and billing teams, as well as with the diagnostics team.

WHAT YOU WILL DO

Manages the client intake process, which includes, but is not limited to fielding incoming requests for services, being the first point of contact for potential families / caregivers, responding to all intake communication and correspondence (phone, email, online application, and fax);

responds to service inquiries in the Signify portal for the TAP program.

  • Provides each potential family / caregiver and existing family / caregiver with a high touch experience, accurately describing the process for client intake on onboarding, answering questions, and providing useful resources as necessary.
  • Coordinates with medical professionals to obtain required documentation, like reports and prescriptions for services.
  • Manages all client onboarding documents including billing registration forms, ROIs, consents, and other intake paperwork, ensuring that it is housed in the client’s electronic medical record, focusing on HIPAA compliance in all aspects.
  • Supports the client onboarding process by managing the ABA waitlists across TAC, identifies new clients, tracks client progress through the onboarding pipeline process and acts as a facilitator for speed to service in conjunction with the clinical team and the billing team.
  • Works closely and collaboratively with the Director of Diagnostics and the diagnostics team to track the completion of intake paperwork and consents, schedule appointments, maintain the waitlist for autism screenings and diagnostic evaluations, obtains any medical records, other clinical documents needed for case reviews and sends completed reports.
  • Maintains accurate documentation associated with the client pipelines and onboarding steps, analyzes data, and provides accurate metrics for speed to service.
  • Comfort in working very near others, within a few feet or less, involving close contact with clients and BBP; may involve conflict resolution opportunities

WHAT YOU WILL BRING

  • Associate’s degree in psychology, sociology or other human services related field
  • Minimum of two (2) years working within a human service setting.
  • Ablity to lift, push, pull and carry a minimum of 25 pounds at a time.
  • Ability to bend, twist or stoop on a repetitive or continuous basis
  • Operational knowledge of various business instruments including computers, iPads, copiers or fax machines requiring manual dexterity.
  • Document analysis and organization may require extended levels of concentration as well as visual acuity to read paperwork and computer monitors.
