Vision Homecare Services is Hiring a LPN's needed for Private Duty Nursing Near Bronx, NY

POSITION: License Practical Nurse (PDN) (8am - 8pm Tuesday - Friday)

REPORTS TO: DOPS/Registered Nurse

POSITION SUMMARY:

The Private Duty Nurse provides nursing care and services to clients in their place of residence/facility in accordance with the agency’s policies and procedures and under the supervision of a Registered Professional Nurse.

QUALIFICATIONS

1. Graduate of an approved LPN Program

2. NYS Licensed and currently registered as a Licensed Practical Nurse

3. MUST have previous Home Health or Long-term Care experience

4. MUST hold a current CPR Certification

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to perform mathematical operations as related to medication calculations, units of American money and weight measurements, volume, and distance.
  • Good organizational writing, communication, and interpersonal skills.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.
  • Ability to deal with problems involving several variables in standardized situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

EQUIPMENT OPERATION

  • Durable medical equipment and nursing equipment.

SPECIFIC DUTIES AND RESPONSIBILITIES FOR THE PRIVATE DUTY NURSE

1. Home Care:

- Provide skilled nursing care to the pediatric patient in a home setting.

- Administer medications, treatments, and therapies as prescribed by the physician.

- Monitor and document the patient's vital signs, progress, and any changes in their condition.

- Assist with activities of daily living, including bathing, dressing, feeding, and mobility support.

- Collaborate with the patient's family members to educate and train them in managing the child's care needs.

- Maintain accurate and up-to-date medical records, including nursing notes and care plans.

- Communicate effectively with the healthcare team, including physicians, therapists, and other specialists involved in the patient's care.

2. School Care:

- Accompany the pediatric patient to school and ensure their safe transportation.

- Collaborate with school staff, including teachers and administrators, to develop an individualized care plan.

- Monitor the patient's condition and provide medical interventions, as necessary, during school hours.

- Administer medications, treatments, or therapies according to the prescribed schedule.

- Maintain open communication with school personnel regarding the patient's health and any changes in their care plan.

- Assist with any necessary accommodations or modifications to support the child's participation in academic and social activities.

SPECIALIZED SKILLS AND TECHNICAL COMPETENCIES:

1. Knowledge of client assessment procedure.

2. Knowledge of the client teaching process.

3. Knowledge of disease processes.

4. Knowledge of medication administration, via oral, IM, and SQ.

5. Knowledge of medication interactions (drug-drug, drug-food).

6. Knowledge of the principles of therapeutic diets.

7. Knowledge of infection control practices, and aseptic techniques.

8. Knowledge of catheter insertion and care.

9. Demonstrates ability to effectively communicate information with clients of all ages and social economic levels.
