Job Overview

A Lead Case Manager’s primary goal is to ensure that an individual reaches their optimum level of wellness and functionality while at Virtue At The Pointe Recovery Center.LeadCase Manager’s assess the needs of the individual and family and coordinate care services through direct contact with the referent. This can include nutrition, rehabilitation, transportation, and educational services. A Lead Case Manager also works to ensure that care is provided in the most comprehensive and cost-effective manner for the individual and their insurance providers. Lead Case Manager’s then monitor the continuity of care and serve as an advocate for the individual. The Lead Case Manager will train and oversee any future case managers hired to assist with client care.

Job Functions

• Provide clinical supervision/support under clinical leadership’s direction to VATP case managers to ensure they are giving the best care to clients they serve at VATP
• Facilitate case management huddles, assign clients, oversee case coverage, care practices, compliance, provide consultation and identify training needs of VATP case management program
• Work as part of an interdisciplinary, integrated treatment team and participate in treatment team meetings to best assure continuity of care between clinical/medical staff and the needs of the client
• Train and oversee any future case managers hired to assist with client care
• The Lead Case Manager is also expected to carry a condensed primary caseload of clients with co-occurring mental health, substance use, medical and psychosocial needs in a high intensity outpatient setting that includes wrap around sober living housing
• Begin working with client upon admission, monitor client’s progress through treatment and maintain support through discharge
• Assist clients with any specialized needs they may have while in treatment i.e. paying bills, family coordination, resolving legal issues, securing transitional housing at discharge, and other direction as given from treatment team providers

Requirements

• Valid Driver’s License with clean driving record, as determined by Virtue’s insurance carrier
• Must hold a Bachelor’s Degree in social work, behavioral health or other social services field
• Certified Alcohol & Drug Counselor (CADC-I) and/or Qualified Mental Health Associate (QMHA-I) credential required
• Minimum 2 years of behavioral health direct patient care and or case management experience required
• Leadership experience in a clinical or medical care environment
• Experience and/or passion for working with Veterans a plus
• Must be 21 years of age
• Knowledge of Windows, basic email, Google drive, and basic computer navigational skills.
• Ability to write professional correspondence and reports free from grammatical and spelling mistakes.
• Ability to read, analyze, and interpret emails, letters, policies, procedures, etc.

• Professional and/or personal experience with mental health and substance abuse recovery highly preferred
• High level of patience
• Active CPR and First Aid certification required
