Position Summary:We are seeking a motivated and compassionate Customer Support Specialist to join our team. This role is vital to the expansion of the Chronic Care Program, focusing on communicating with potential patients, primarily the elderly, about the benefits of our program. The successful candidate will be responsible for making outbound calls to prospective patients, discussing the features and advantages of our program, obtaining patient consent, and facilitating their enrollment. This position requires excellent communication skills, a passion for helping others, and a talent for presenting our program in a compelling and respectful manner. Key Responsibilities: • Make outbound calls to potential patients to introduce the Chronic Care Program. • Clearly and effectively communicate the benefits and features of the program, tailored to meet the needs of the elderly population. • Obtain informed consent from patients for enrollment in the program. • Accurately complete patient enrollment processes and document interactions in our system. • Maintain a high level of professionalism and empathy during all patient interactions. • Meet or exceed enrollment targets while ensuring a positive experience for each patient. • Provide feedback from patients to help improve the program and enrollment process.
