What does PARTNERSHIP look like?
RN Care Management is responsible for care coordination of each client receiving services from Virginia Home Care Partners. A critical component of the RN Care Management role is to demonstrate the company’s culture of compassion, accountability and results by displaying the highest standards of clinical integrity, superb follow-through and consistency, and exceptional customer service and client satisfaction. RN Care Management works in coordination with the Care Support Manager and Administrator to ensure client care, compliance and Care Team Support is maintained with excellence.
