Registered Nurse, RN - Hospice Staff

Virginia Health Services is hiring a Hospice Staff RN, to work on the Peninsula in the Newport News and Williamsburg area. Currently hiring for a Full Time position.

The Hospice Staff RN has the following responsibilities:

  • Regularly assessing and reassessing the nursing needs of the Hospice patient.
  • Providing Hospice nursing services, treatments and preventive procedures.
  • Providing dietary counseling.
  • Initiating nursing procedures appropriate for the patient's Hospice care and safety.
  • Observing signs and symptoms and reporting to the physician and IDG members any unexpected changes in the patient's physical or emotional condition.
  • Teaching, supervising and counseling the Hospice patient and family members about providing care for the patient.
  • Supervising and training other nursing service personnel.
  • Developing and re-evaluating the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
  • Performing specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders.
  • Attending team conferences.
  • Maintaining records as required by Hospice.
  • Following the policies and procedures of Hospice. Observing confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
  • Always communicating to the supervisor if unable to meet a patient's need or perform a procedure.
  • Participating in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
  • Coordinating the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
  • Complying with agency infection control policies and protocols.
  • Assisting with orientation, teaching and training as requested.

Qualifications


QUALIFICATIONS AND REQUIREMENTS: 

  • Graduate of an accredited School of Nursing and holds current Virginia RN license.
  • Minimum of 2 years experience as a registered nurse, preferred.
  • Active patient contact within past three years, preferred.
  • Ability to work independently, make accurate, and at times, quick judgments.
  • Ability to supervise others appropriately.
  • Ability to respond appropriately to crisis outside of a hospital setting.
