Job Summary
The Social Care Supervisor (SCS) is a member of the Behavioral Health/Mental Health Department, supporting the Primary Care Clinic team to assess and address patient Behavioral and Mental Health needs with a focus on patients with complex care needs including Transitions of Care and high ED Utilization. The SCS will be the hub in each clinic that receives information and support from the Transitions of Care Team and disseminates across the clinic providing leadership in the development of a Care Plan for patients with various complex needs. Specific responsibilities include Supervision of Social Care Navigators at the clinic and the patient care plans, provide basic health education, system navigation, case management and skills training, function as an advocate and coach for patients to help them pursue healthy lifestyle changes with an emphasis on self-determination, patient engagement and self-efficacy. The SCS utilizes modalities within the scope of their degree and licensure including counseling, assessment and collaborative treatment planning.

The SCS will supervise the Social Care Navigator(s) at their primary location or as part of the broader County team providing direct supervision and guidance as well as administrative supervision of timesheets, annual performance reviews and coordinating onboarding and required training.

Essential Duties and Responsibilities:

· Provide individual patient assessments used in the development of comprehensive integrated medical care services including Behavioral and Mental Health.

· Provide skilled social work services including education and instruction for patients and caregivers supporting the delivery of ongoing care.

· Coordinate and modify plans of care with the patient, caregiver, primary physician, social care navigators and other members of the inter-disciplinary health care team.

· Evaluate patient health care needs in consultation with the primary physician for the purpose of making appropriate referrals to other community agencies and services as deemed clinically appropriate.

· Attend community meetings that contribute to the development of resources for patients.

· Assist with coordination of patient referrals from hospital care departments, including outreach, reviewing/tracking inpatient stays, and follow-up in order to support social service needs.

· Uses Motivational Interviewing to assess and positively affect the patient’s level of engagement in his or her care and confidence in his or her ability to carry out a self- management plan.

· Provide care for all patients utilizing appropriate infection control precautions and practices, standard and transmission-based precautions, including patients with communicable diseases.

· Incorporate into patients medical record timely clinical documentation in compliance with regulatory and agency guidelines.

· Initiate and participate in patient care inter and multi-disciplinary case conferences and educational and program service programs.

· Supervise BSW/MSW/LPC intern(s) to enhance educational and professional learning

· Provide regular individual and group clinical consultation to paraprofessional staff contributing to Transitions of Care and Complex Care work.

· May perform community based (home, hospital/ED) visits and/or patient accompaniments when indicated, following Home Visit Safety Guidelines.

· Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

· Other duties as assigned by supervisor.

HIPAA Requirements:
The person holding this position will have access to PHI during the course of his/her work activities. He/she will use the information to provide individual patient assessments to be used in the development comprehensive integrated medical care services. Applying the minimum necessary standard of HIPAA, the designated record sets to which an employee holding this position will have access include: All sections of the medical record, patient demographic information in the practice management system, incoming records, reports, results, consultations, etc. The SCS is required to read the content of these records only to the extent needed to accomplish the assigned tasks.

Required Knowledge, Skills and Abilities:

· Knowledge of methods of behavioral medicine and evidence-based treatments for medical, mental and behavioral health conditions.

· Knowledge of the problems and dynamics in treating medically underserved populations.

· Thorough knowledge of use of common office equipment (copier, fax, printer).

· Excellent oral and written communication skills.

· Ability to work successful primary health care team member.

· Use of personal vehicle or other transportation is required for travel to multiple locations within Washington and Yamhill counties.

  • Knowledge and experience in working with computer systems and electronic medical records.
  • Knowledge and experience working within Epic as the electronic medical record.
  • Knowledge of Medicare and Medicaid.
  • Knowledge of adult learning and behavior change principles.
  • Knowledge of community-based resources.
  • Demonstrated skills in motivational and health coaching, patient advocacy, health promotion, disease prevention, patient education and resource management.
  • Bilingual Spanish strongly preferred.

Supervision / Supervisory Responsibilities:

· Master’s degree in Social Work (MSW), Psychology, or related field required.

· 2 years’ experience in supervision of interns, students, and social service staff preferred.

· Experience in facilitating and consulting with interdisciplinary teams.

· Experience in providing supervision for behavioral and social service staff.

Education Experience and Training:

· Master’s degree in Social Work or Psychology from an accredited school required.

· Demonstrated knowledge of community health, welfare, and social agencies is required.

· Commensurate with educational preparation, demonstrates the ability to apply age specific principles of growth and development and life stages to meet each patient’s needs.

· Experience working in a multi-cultural, diverse environment.

· This position requires a minimum of one-year relevant experience. Experience in one or more of the following areas strongly preferred (Social Work/LPC internships count as experience):

o Care Management

o Behavioral Health

· Social Services Registered with Board as under supervision towards licensure preferred.

· BLS required.

· A valid state driver’s license, transportation and auto insurance is required or alternative means of transportation.

· Experience in Community Health Care working with indigent populations.

Physical Requirements:
