Salary: $50,000- 60,000 / year depending on licensure
The Recovery Support Navigator (RSN) provides intensive care coordination for MassHealth Members with complex medical, behavioral needs along with a history of Substance Abuse Disorder (SUD) and are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan.
The RSN will collaborate with the Community Partner (CP) team and the clinical staff of each Enrolllee's ACO/MCO plan in order to minimize duplicative efforts in order to promote integrated care, and to ensure the quality and continuity of care. The RSN specializes in supporting Enrollees with SUD and serves as the team's resource on SUD assessment, treatment and rehabilitation techniques and resources. The role supports the Team Leader in maintaining the team's adherence to fidelity standards of co-occurring disorder (COD) evidence- based practices. This role drives outreach and engagement, assessment and care planning, care transitions, health and wellness coaching, as well as community and social services connections in partnership with Enrollees and their care teams.
