Vine Homecare, Inc. is Hiring a LPN OR RN FOR HOME HEALTH AIDE SUPERVISORY VISIT Near Danvers, MA

JOB SUMMARY:

Responsible for all aspects of patient care management, including day-to-day supervision of direct patient care provided by Home Health Aide and office personnel supervision.

Responsible for quarterly and bi-weekly supervision of Home Health Aide.

JOB RESPONSIBILITIES:

  • Supervises all professional and non-professional staff to ensure quality performance standards.
  • Makes the placement visit for HHA new cases or when assigned aide changes
  • Oversees daily Agency operations and implementation of policies and procedures.
  • Performs the skills competency check prior to the new aide first visit.
  • Oversees and assures the delivery of all patient care and services according to professionally accepted standards of practice.
  • Oversee the development and implementation of patient care plans for assigned patients.
  • Review and approve HHA care plans, ensuring they meet patient needs and are implemented by the HHA's.
  • Coordinates with case managers in decisions regarding patient eligibility and suitability for home care services.
  • Receives and resolve complaints made by patients/families regarding care or services, personnel or policies.
  • Evaluates, and regularly re-evaluates the needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care. Assesses the patient's continual care needs.
  • Performs home safety check and environmental check of the client’s home environment. Reports any unsafe or potentially unsafe condition to the administrator.
  • Assists with and coordinates training of appropriate staff and conducts performance evaluations of workers to ensure quality care, compliance, and fiscal responsibility.
  • Coordinates regular case/team conferences and promotes communication of information relevant to the patient care process to ensure quality of care and coordination of services.
  • Participates in agency quality assurance activities designed to improve quality and continuity of patient care.
  • Acts in the place of the Administrator when the Administrator is not on site.
  • Mentor and train HHAs on policies, procedures, and best practices.
  • Address staff concerns and ensure a positive and supportive work environment.
  • Recruit and interview potential HHA candidates.
  • Ensure compliance with all agency policies, procedures, and regulations.

Qualifications include:

  • Must posses a current active license as a Licensed Practical Nurse (LPN) or RN in Massachusetts.
  • Must possess current valid driver's license.
  • Must be able to travel to patients' place of residence without restriction as necessary.
  • Ability to work independently and manage time effectively.
  • Must be able to stoop and bend; must be able to lift and transfer patients.
  • Two years’ experience of supervisory experience preferred but not required.

Job Type: Full-time

Pay: $33.00 - $40.00 per hour
