Vienna hospice is currently seeking a Hospice Licensed Vocational Nurse (LVN) to work PER-DIEM. We are looking for people who will go above and beyond to make sure that direct patient care under the supervision of a Registered Nurse is top notch. Responsibilities:

• Providing skilled nursing patient care under the supervision of a registered nurse.
• Follows the plan of care and follows the comprehensive assessment
• Provides accurate and timely documentation of patient services to reflect on the plan of care
• Administers medication and treatments as ordered by the physician
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Reports the ongoing changes about the patient to the interdisciplinary team, includes, registered nurse, hospice aid, and DOPCS.
• Able to perform other duties as assigned by the RN or DOPCS.

Qualifications:

• Has a California State license as a Licensed Vocational Nurse
• A minimum of (1) year experience with Hospice.
• Has experience with Hospice MD
• Demonstrates good verbal and written organization skills.
• Must be a California licensed driver with an automobile.
• Has a flexible working hour schedule and be available to work on weekends or continuous care shifts; if needed.
• Understands the philosophy of hospice concept and the needs of terminally ill patients.
