Rapid growth and expansion has created an opportunity for an experienced PRN LVN.Job DescriptionReporting directly to the Director of Nursing, this licensed, compassionate and technically savvy individual delivers the best possible patient care by caring for ill, injured, convalescent, or disabled persons in their homes. Under the direction of the RN assists in identifying the patient’s physical, psychosocial and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations. Work as part of a health care team to educate patients and their families about program offerings, assess patient needs, plan and modify care and implement interventions.QualificationsPosition Functions•Under the direction of the RN assists in identifying the patient’s physical, psychosocial and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations.•Participate in planning, implementing care in conjunction with the RN, according to the plan of care.•Observe patients, charting and reporting pertinent changes in patients’ conditions, responses to interventions, treatments, and taking any necessary action including contacting doctors and other authorized personnel asappropriate.•Collect laboratory samples as ordered•Maintain HIPAA compliance at all times•Submit necessary reports and required patient care documentation timelySoft Skill Requirements•Excellent Communication skills, both oral and written•Compassionate and caring attitude, ability to deal with people from a wide range of backgrounds, ages, and temperaments•Ability to use discretion concerning sensitive information and adhere to HIPAA•Ability to work on own initiative within clearly defined parametersEducation and Experience•Tx Nursing License•Experience using required equipment like hypodermic needles, nebulizers and accessories, patient care beds and accessories for specialty care, fall prevention devices and accessories,Additional Information All candidates will be required to pass a criminal background check.
