The Job

Primary responsibilities for the CRNP will include providing advanced primary and/or specialty care to patients primarily in patient homes. Duties will include, but are not limited to, disease management, performing physical exams, recording patient medical histories, and creating patient care plans that are aligned with patient’s goals. This position involves daily travel.

Essential Duties and Responsibilities:

● Provide extensive primary care to patients, with the goal of helping the patient to age in place as appropriate and to reduce both unplanned hospitalization, unnecessary emergency department visits and nursing home admission.

● Provide Advance Care Planning discussion to illicit patient’s state goals of “What Matters Most” in initial assessment and follow up visits.

● Assist the practice and participates with the Practice Administrator and Home-Based Primary Care Physician in meeting safety, performance, and quality measures for a Home-Based Primary Care Practice.

● Work collaboratively with other Patriot Home Care Services lines and HBPC interdisciplinary team to achieve patient and model goals.

● Time management

● Triage incoming messages, completed within the same day as the message was received.

● Complete all progress notes within 24 hours of the visit.

● Perform physical exams and patient observations.

● Record patient medical histories and symptoms.

● Order, administer and analyze diagnostic tests.

● Evaluate responses to treatments and medications.

● Appropriately prescribing medicines within the scope of prescriptive authority including schedule II-controlled substances.

● Use of most appropriate and safest approaches to pain management using non-narcotic approaches first when indicated.

● Diagnose health issues.

● Detect changes in a patient’s health, change in condition, and change treatment plans as necessary.

● Create patient care plans and contribute to existing plans. Meet documentation standards for all visits, chronic care management, advanced care planning, prolonged services and evaluation and management codes.

● Work with the HBPC interdisciplinary team (IDT) to coordinate care plans and services.

● Monitor and operate medical equipment.

● Involve community resources as needed.

● Consult with healthcare professionals.

● Train patients and families on managing illnesses or injuries and disease prevention.

● Maintain quality, productive relations with other Patriot Home Care staff, and vendors.

● All other duties as assigned.

● Must have prescriptive authorization.

● APN authorization in the state where employed to practice in Family Health by the Board of Nurse Examiners

● Reliable transportation,

● Valid driver’s license with automobile insurance

The Qualifications

Education:

Completed an accredited, Board-approved master’s or postmaster’s Nurse Practitioner Program or other Board-approved program that awarded an advanced degree; or a course of study considered by the State Board to be equivalent to that required for certification at the time the course was completed.

Credentials/Licensure:

· RN licensure in State of Delaware

· Certification from Nurse Practitioner Program

· Valid driver’s license

Experience:

· 3 years of experience as a Nurse Practitioner preferably in a skilled home health, PACE or hospice setting

· Proven experience providing care to an older adult/elderly population.
