Veterans Recovery Resources is Hiring a Family Nurse Practitioner Near Mobile, AL
- We are looking for a Family Nurse Practitioner to join our team and provide family-focused care in our organization.
- Offer person-centered treatment planning with personally and culturally diverse clients.
- Conduct physical examinations and coordinate specialized medical services as needed.
- Obtains patient history and performs assessment via telehealth, direct observation, interview and examination.
- Performs preventative health assessments, screening, immunizations and patient care.
- Obtains appropriate patient history, orders appropriate diagnostic tests and follows-up with interpretation, formulates assessment and initiates treatment plan with appropriate follow up appointments.
- Reviews incoming reports (e.g. lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner.
- Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.
- Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy
- Provide medication therapy management for primary care diagnoses, mental health disorders, and other health-related conditions in accordance with scope of practice and clinical standards of care.
- Monitor effects of medications (e.g., metabolic syndrome) based on standards of care.
- Participates as an active member of the medical staff and participates in daily care team huddles to ensure prioritization of patient needs.
- Support operations via consultation and service planning.
- Outreach to regional medical centers/clinics to ensure proper working relations.
- Graduation from an accredited program of advanced practice nursing.
- Current licensure as a Certified Registered Family Nurse Practitioner (ANCC or AANP)
- Licensure with the state of Alabama as a nurse practitioner and registered nurse.
- At least two years (4,000 hours) of experience in a primary care setting is required.
- Current BLS required.
- Background in working with co-occurring conditions, trauma, and SUDs is preferred.
- Background in working with inter-disciplinary behavioral health teams is preferred.
- DEA registration required within 180 days of employment.
- Ability to conduct physical examinations and coordinate care with other providers.
- Ability to provide medication therapy management.
- Ability to monitor adverse effects of medications.
- Excellent interpersonal skills and ability to build trust with other members of the inter- disciplinary team and community partners.
- Knowledgeable about cultural humility, trauma-informed care, evidence-based practice, military cultural competence, acceptance, and commitment therapy (ACT), peer support, and their psychosocial approaches to recovery and psychiatric rehabilitation.
- Willingness to commit to VRR’s organizational values (e.g., compassion, justice, and equity) and operational principles (e.g., holistic, person-centered, evidence-based care)
- Dual reporting to Medical Director and Nursing Director
- Local travel in Mobile and Baldwin Counties (estimated 50 miles per month)
- Reliable transportation and state-mandated minimum vehicle insurance coverage
- Salary Exempt
- Full-time position at 1.0 FTE
