Veterans Health Administration is Hiring a RN- Geriatrics- Long Term Care Near Albany, NY

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Qualifications:

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
    • For Applicants enrolled in MSN Bridge or Non-traditional courses of study: The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience:
  • One year RN experience on a long-term care unit
  • One year acute med/surg RN experience
  • Previous experience with MDS
  • American Heart Association Basic Life Support for the Healthcare Provider
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.

Physical Requirements: Heavy Lifting (35 lbs and over); Heavy Carrying (35 lbs and over); Straight Pulling (up to 1 hour); Pushing (1 - 2 hours); Reaching above shoulder; Use of fingers; Both hands required; Walking (up to 4 hours); Standing (up to 4 hours); Kneeling (up to 2 hours); Repeated bending (up to 4 hours); Both legs required; Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13' to 16'; Far vision correctable to 20/20 in one eye and to 20/40 in the other; Ability to distinguish basic colors; Ability to distinguish shades of colors; Hearing (aid permitted); Emotionally/mentally stable

Responsibilities:

The staff nurse is a registered nurse responsible for continuous delivery of patient care. This position potentially requires flexibility in schedule and assignments, as this is a 24/7 hospital. The successful candidate must be poised and articulate in communicating with people of varied educational levels and varied ethnic backgrounds. Able to work cooperatively as a member of a team in all assignments. Makes caring for the Veteran his/her priority while in the work setting and demonstrates customer service principles in all aspects of work and is self-motivated in the pursuit of meeting professional work standards. The responsibilities of the RN include but are not limited to :
  • Completes and documents nursing assessments seeking assistance to identify priority needs
  • Demonstrates knowledge and skills necessary to provide care appropriate to the residents served and interprets information to identify each resident's individual needs
  • Follows resident's personal requests and needs based on resident's minimum data set care plan
  • Assigns and directs activities of supportive personnel (i.e. Nursing Assistants, LPNs, MSAs, etc.)
  • Provides direction for team members in meeting patient care needs
  • Develops the initial nursing plan of care and updates plan
  • Addresses resident safety needs, following National Resident Safety Goals when communicating resident care needs to other nursing staff and disciplines.
  • Includes patient/family in establishing realistic, measurable health goals, and discharge planning ,seeking assistance when needed.
  • Implements and delegates nursing interventions/orders and physician's orders in a timely and complete manner, requesting assistance when needed
  • Safely and effectively uses equipment required for resident care, including procedures for cleaning, transporting and storing reusable medical equipment
  • Evaluates resident status
  • Recognizes overt abnormal signs and symptoms or changes in residents needs and relays information to geriatric MD and documents reported changes with name of geriatric physician.
  • Follows up appropriately with change in resident's condition and identifies priority of action
  • Evaluates and documents residents response to interventions and progress toward attainment of goals/expected outcomes
  • Documents care given in the medical records in a consistent, accurate, concise and timely manner
  • Evaluates and documents resident response to interventions and progress towards attainment of goals/expected outcomes. Revises plan of care according to evaluations
  • Serves as advocate for resident concerns and treats residents with respect
  • Maintains confidentiality of all resident information including verbal, electronic, and printed forms of communication
  • Makes resident and family comfortable during CLC stay, especially during the resident's end of life
  • Identifies need to seek assistance in the delivery of care
  • Works effectively with professional and support personnel who provide resident care
