Veterans Health Administration is Hiring an Advanced Register Nurse Practitioner - Geriatrics & Extended Care Near Beach, FL

The VA West Palm Beach Healthcare System provides patients with outstanding health care, trains America's future health care providers, and conducts important medical research. We deliver primary and specialty health care services to over 57,000 veterans throughout South Florida and along the Treasure Coast. The incumbent will execute responsibilities that demonstrate leadership, experience, and creative approaches to manage complex patient care within Geriatrics & Extended Care Service.

Qualifications:

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Preferred Experience: Experience in community care, hospice and geriatrics.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level III - Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: Lifting up to 35 pounds for 8-12 hours, intermittently; moderate carrying (15-35 pounds) for 8 to 12 hours, intermittently; lifting over 35 pounds with assistive devices in accordance with the Safe Patient Handling Policy except in emergent situations requiring prompt patient care; pulling for 8 to 12 hours, intermittently; pushing for 8-12 hours, intermittently; reaching above the shoulders for 8-12 hours, intermittently, both hands required; walking 8-12 hours, intermittently; standing 8-12 hours intermittently; kneeling 8 to 12 hours, intermittently; repeated bending for 8-12 hours, intermittently; climbing and use of both legs and arms for 8-12 hours, intermittently, both legs required; near vision at 13 to 16' Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other eye; ability to distinguish basic colors; hearing aid permitted; twisting, simply grasp and fine manipulation.
The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

Responsibilities:

The Geriatrics & Extended Care Service (GEC) Nurse Practitioner is responsible for promoting the veteran's maximum level of health and independence by providing comprehensive care and optimizing physical, cognitive, and psychosocial function of the Veteran in various settings. Thus reducing the need for, and providing an acceptable alternative to, hospitalization, nursing home care, emergency department and outpatient clinic visits, through longitudinal care that provides close monitoring, early intervention, and a therapeutic safe home environment.

Additional duties may be assigned by the Primary Supervisor Physician, appropriate Chief of Service or Chief of Staff, which may included provisions for special training to assure proficiency.
  • Demonstrates advanced skills, knowledge, and an ability to perform treatments, procedures and utilize equipment appropriate to practice
  • Provides direct care and/or care via telehealth as appropriate to adults with medical issues, including assessment, diagnosis, and treatment of health care problems which may include prescription of pharmacological and non pharmacological interventions according to the VHA standards
  • Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care problems
  • Documents appropriately patients' History and Physical, as well as requesting appropriate consults
  • Independently manages patient care and collaborates with other members of the interdisciplinary health care team as necessary
  • Revises the treatment plan based upon patient assessment and diagnosis
  • Uses the Computerized Patient Record System (CPRS) per policy
  • Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes
  • Maintains professional boundaries with patients, families, and employees. Behavior as a role model is transparent and above reproach
  • Effectively manages assigned resources and identifies creative strategies to increase efficiency within the unit/team/workgroup
  • Anticipates and recognizes medical/surgical/psychiatric emergencies and initiates or leads others to implement appropriate and timely actions
  • Assists nursing personnel with assessment, implementation, and evaluation of new approaches, procedures, and standards of patient care, based on reliable nursing research and/or literature review
