We help these individuals live happier, healthier lives by partnering with their aides and caregivers, as a key part of the care team. We use a combination of virtual care, home-based and mobile technologies, data integrations and partnerships with home care agencies to make the home an integrated setting of care with patients, and their Caregivers at the center.

Vesta is the Roman name for the goddess of home, hearth and family. She is the caregiver. Often unseen yet greatly revered, she puts others' needs ahead of her own, keeping the hearth warm so the home and family can function.

We see Caregivers and recognize the power and potential they embody. More than just assistance, Caregivers are eyes, ears and hands in the home. Caregivers play the role of Doctor, Nurse, Pharmacist, EMT and more, but without support or guidance. That is where Vesta Healthcare comes in. Our program provides Caregivers with a personalized clinical team in their pocket. Our team links Caregivers to the people they care for and the other providers involved in their care. It's an insurance covered benefit, so it's available to most adults with Caregivers free of charge to them.

The ideal teammate would be…
Someone who will be the "on the ground" liaison for local level agency staff in account management and training. They will be responsible for the daily review of client dashboard metrics to determine directional trends of KPIs. This person will oversee the management of client work plan activities and deliverables. They will conduct ongoing client implementation support including introduction/orientations and relationship growth/management. They are capable of building a shared understanding of a problem or opportunity amongst a diverse group of project participants; and can foster and manage healthy conflict. This is an in person role and would require reliable transportation to meet with clients in/around New York City weekly.

The ideal teammate will be able to:

  • Build effective strategies, tools, and processes to deliver network census growth (Vesta Healthcare and Agency) and increased value to our customers
  • Develop and maintain levers associated with the network of agencies including evaluation of network status, growth and retention
  • Maintain clinical and operational coordination with agency staff, communicating reports to local agency leadership on individual member escalations, member recertification/waiver coordination, complaint resolution, and nurse management program activity
  • Lead internal clinical reviews to categorize avoidable and unavoidable readmissions by creating plans to minimize avoidable readmissions while crafting processes for regular data access and exchange
  • Shape and prioritize agency registered nurse manager trainings; driving internal process that focus on improving patient quality of care without increasing member costs
  • Support the onboarding of new agencies by coordinating and integrating the Vesta Healthcare program into marketing, enrollment, patient engagement, and retention workflows
  • Train agency staff and local leadership on benefits, talking points, and means of coordination around the Vesta Healthcare program
  • Continue providing regular executive and operations cadence with business leaders within each agency around monthly performance and quarterly steering.
  • Ensure overall success of business relationships with agencies in tracking, reviewing, and developing action plans based on KPI's for each agency including Enrollment, Engagement, clinical, patient satisfaction/retention and financial results

Would you describe yourself as someone who has:

  • 3 years of healthcare/managed care B2B and/or MLTC experience (required)
  • 3 years in client management/Business Development experience (required)
  • Live in or around New York City and have reliable transportation to work in the field meeting clients (required)
  • Familiarity with managed care plans/structure in New York State (required)
  • Strong organizational skills, including the ability to prioritize and lead several complex projects simultaneously
  • Passionate about our mission to improve people's lives
  • Comfortable in a dynamic and always evolving start-up environment

Pay range is $100,000-$110,000 (plus referral bonuses) based on experience. (The referenced salary range is based on the Company's good faith belief at the time of posting. Actual compensation may vary based on factors such as geographic location, work experience, market conditions, education/training and skill level).
