Job Summary: We are seeking a compassionate and dedicated Registered Nurse who can work PRN to join our team. In this role, you will provide high-quality care to our patients on an as-needed basis. This position is ideal for a nursing professional looking for flexible work opportunities while contributing to a dynamic and patient-focused environment.

Key Responsibilities:

• Monitor patient status and report any changes to the healthcare team.
• Educate patients and families on health conditions, treatment plans, and post-discharge care.
• Collaborate with a multidisciplinary team to achieve the best patient outcomes.
• Maintain accurate and thorough patient records.
• Complete monthly assessments and advise direct support professionals
• Travel to individual’s home

Qualifications:

• Current and valid RN license in the state of Indiana.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 1 year of nursing experience
• BLS/ACLS certification required.
• Strong clinical skills and the ability to make decisions in a fast-paced environment.
• Excellent communication and interpersonal skills.
• Flexibility to work various shifts, including days, nights, and weekends.
