Position Title: Personal Care Assistant

FLSA Status: Non-Exempt

Reports to: Registered Nurse Manager

Pay Scale: E

Department: Vernon Acres Senior Living

I. Position Summary This is a resident care associate position at Vernon Acres Senior Living where there is an emphasis on person-centered care and a home-like environment. Employees work under close to moderate supervision of the Vernon Acres Senior Living Nurse Manager. Employees are responsible for assisting tenants with a variety of needed services, including direct supportive and personal care, assistance with activities of daily living, medication distribution, documentation and reporting, implementation of activities, meal service and laundry / housekeeping duties as assigned. When possible, encourage the highest practicable level of independence.

II. Essential Duties and Responsibilities • Assists tenants in daily activities, such as orientation of new tenants, escorting and transporting to various activities, encouraging socializing and communication with other tenants, and assisting and encouraging them with self-care to preserve their dignity and self-worth. • Assists with or provides personal care. transfers tenants using proper body mechanics or gait belt or as necessary. Adheres to and follows individual tenant plans of care. Documenting exceptions to the individual plan of care, any changes or concerns in tenant status or any additional tasks completed for the tenant. • Record incidents, errors, or accidents and record progress notes in electronic medical record. • Checks alarms/devices for proper operation. Answers emergency call system and follow up as necessary. • Ability to think, act, and intervene independently in both routine and emergency situations. • Be self- motivated and function as an” in charge person”. Considerable initiative, judgment, and leadership skills are also required.• Relate to tenants, staff, and visitors in a courteous and diplomatic manner under all circumstances. • Performs simple nursing duties such as weights, temperatures, pulse, respiration, and blood pressure. • Shows accurate power of observation, anticipating the needs of the tenants, noting any changes in the physical condition and cognitive/emotional status and reports findings to the Nurse Manager. • Respond promptly and accurately to the tenant’s needs and requests within their limitations, reporting promptly and accurately any changes to the Nurse Manager. • Participates in defining needs of tenants to assist in development of the tenant’s plan of care. • Determines and administers medications by following the “Five Rights”: (Right tenant, Right medication, Right dosage, Right time, and Right route) • Assists with meal service and dining (escorting tenants to and from dining room, set up and clean tables, serve meals, bus tables) • Changes bed linens, collects laundry and assists with light housekeeping duties. Empty tenant garbage regularly and common areas as assigned. Assures tenant apartments are neat and safe. • Documents and records personal items of tenant upon admission, daily care, activities and behavior, any injuries or suspected tenant abuse, and submits reports in a timely manner. • Works with Nurse manager to implement planned recreation / social programs and to engage tenants. • Responsible for participating in and supporting the resident-centered activity program. Assist in reminding/escorting residents to and from activities. • Observes good hygiene practices including handwashing between tenant cares and maintains good personal hygiene. • Understands universal precautions for infection prevention and control and follows these procedures while doing resident care. • Works safely and follow safety practices and standards of the facility; responsible for reporting or correcting any existing or potential safety or accident hazard. Monitor for environmental safety hazards. • Notifies maintenance of any building concerns and update Nurse manager as needed. • Maintains tenants; rights and confidentiality of tenant information; reports all suspected cases of resident abuse to the Nurse Manager.• Follow up with appropriate staff, tenants or other individuals regarding reported complaints, problems, and concerns. • Be aware of and adhere to Tenant Rights • Shows concern for tenants and displays sincere interest in welfare and comfort of the tenants. • Demonstrates friendliness, ambition, patience, courtesy, thoughtfulness to others, honesty, optimism dependability, professionalism, confidentiality, and good work ethic. • Participates in and completes in a timely manner, staff development programs, in service educational programs offered at Vernon Acres Senior Living • Responds to and acts appropriately in emergency or disaster situations according to current policies and procedures. Report all emergencies to the appropriate individual(s). • Know fire and disaster procedures. Demonstrate knowledge of responsibility in care and safety for residents and co-workers during drills and alarms. Maintain knowledge of fire and disaster procedures for Vernon Acres Senior Living. • Answer phones and directs calls, greet visitors and perform some clerical duties as necessary. • Employee is expected to report to work when scheduled. Possess a high level of flexibility and tolerance to adjust to changes, workloads, assignments, hours, and days (i.e., weekends, evenings, and holidays). • In this public service position employee is required to be positive, courteous, cooperative, and respectful at all times, with the public and tenants. Also establishes and maintains a courteous, cooperative, and respectful working relationship with other employees, supervisors, and public officials. • Perform other duties as assigned.

III. Related Job Functions • May assist tenants to dining areas / common areas. • This job description is not intended to encompass every job duty or responsibility but is only illustrative. This position is required to perform other duties as may be assigned or required.

IV. Physical Demands

Uses near vision, stands, walks, moves throughout the assisted living. Uses hands for delivering personal care. Communicate to tenants and staff, talking and hearing. Ability to kneel or squat within six inches of the floor; push and pull tenants, linen carts, wheelchairs with tenants, beds. Lifts 60 pounds. Writes and reads. Use nursing related equipment such as blood pressure devices, scales; wheelchairs etc.

V. Education and Training Requirements • Knowledge of procedures for personal hygiene, infection control, housekeeping, and clothing care. • Knowledge of techniques for lifting, moving, and transferring tenants. • Knowledge of alarm/ device application policy and procedure • Knowledge of and understanding of and desire to implement Vernon Acres Senior Living Mission and Vision statements. • Ability to effectively relate with the elderly and disabled. • Ability to read and understand written instructions. • Ability to record observations and activities accurately. • Ability to work weekends and holidays, and as scheduled. • Ability to encourage and engage tenants in programs and accommodate to meet their needs. • Training, experience, and other requirements • High school diploma or GED, unless enrolled in School -to- Work program. • For Certified Nursing Assistant, must be on the Wisconsin State Registry for Nursing Assistants with current Federal Registry eligibility. • If not certified, requires previous experience performing personal care duties in an assisted living center or nursing home. • Work history showing dependability, punctuality, and excellent customer service skills. • Preference for work experience in an assisted living center • Speak and write the English language in an understandable manner. • After hire and in a timely manner, completes mandatory in-service trainings. • Must be 18 years of age. • Must pass background check and pre-employment drug screen.
