The Center at CenterPlace is Greeley's premier, dedicated short-term rehabilitation facility featuring 54 private suites, state of the art rehabilitation equipment, and care delivered in a hotel-like setting.

Our center is seeking team-oriented LPNs who are dedicated to patient outcomes and customer service.

ESSENTIAL DUTIES & RESPONSIBILITIES

--Administers and documents the administrations of medications and treatments per physician orders

--Assesses patients routinely and with changes of condition

--Communicates changes of condition, test results, etc. as per company policy

--Educates patients as necessary

--Ensures the patient care plan is implemented

--Supervises and coordinates CNAs
