Ventura Services Florida is Hiring a Social Services Assistant Near Chipley, FL

Washington Rehabilitation & Nursing Center is seeking an experienced Social Services Assistant (SSA) to join our team.

Job Summary:

Under the direction and supervision of the Social Services Director, this position is responsible for implementing the social services function of the facility in accordance with the resident’s needs, government regulations, and company policies as to maintain each resident at his/her maximum potential.

FLSA: Hourly non-exempt

Shift/ Hours: Monday thru Friday, 8:00 am- 4:30 pm

Essential Duties and Responsibilities:

·Ensure the necessary forms, reports, and progress notes are properly completed and contain informative descriptions of the services provided and the residents’ responses to the services.

·Assist in developing for each resident a preliminary and comprehensive assessment, and written care plan that identifies the emotional support and social problems and/or needs of the resident and their goals to be accomplished for each problem and/ or need to be identified.

·As part of the admission process, meet with the resident and family to review policies on advance directives, end of life, resident rights, and smoking policy.

·Provide information to residents and their families concerning available support agencies to meet the needs of the residents on discharge.

·Keep the resident’s family members informed of resident problems, personnel needs, transfers, and changes of level assignment including room changes.

·Participate in assessing, reviewing, and revising care plans as required at least quarterly. Encourage residents to participate in the development and review of care plans.

·Assist with abuse and reportable investigations.

·Review and promptly report to the Social Servies Director and maintain a written records of all resident complaints and maintain the facility log of grievances.

·Assist with weekly psych meetings.

·Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the resident’s return to the community.

·Assist with preparation of Social Services notes and assessment by interviewing residents and family members to gather information.

·Will shop for residents on a weekly and monthly basis which includes monitoring their smoke box.

·Communicate with business office regarding resident funds.

·Maintain a working knowledge of applicable government regulations and ensure compliance.

·Develop and maintain rapport with all services to ensure that a team effort is achieved in developing a comprehensive care plan for all residents.

·Investigates residents’ complaints regarding their rooms, lost items and provides information to the Social Worker for resolutions.

Requirement / Qualifications of the Social Services Assistant

·Prefer a minimum of one (1) year related experience in a healthcare setting.

·Must be high graduate or equivalent.

·Completion of post-secondary education and be able to read and write.

·Effective verbal and written communication skills.

·Must be familiar with medical terminology.

·Must be proficient in the use of personal computer (windows, internet, and applications).

·Strong analytical and problem-solving skills.

·Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.

·Strong attention to detail and accuracy, excellent organizational skills with the ability to prioritize, coordinate and simultaneously maintain multiple projects with a high level of quality and productivity.

Physical Demands:

·This role requires frequent walking, lifting, pushing, and pulling to various locations withing the facility.

·Must be capable of performing the essential job functions of this position with or without reasonable accommodations.
