The Day Services Assistant assists in implementation of the goals and objectives established for clients. This position shall provide continuous training and supervision for assigned clients.
The Employment Coach is directly involved in the process of job training and the provision of support to clients placed into community work sites through insite. Job requirements are highly individualized and based upon client need and placement.
Essential Functions
1. Implements individualized service plans as directed by the Client Services Supervisor.
2. Completes required documentation for clients.
3. Attends continuing educational programs and in-services with recommendation of Client Services Supervisor and approval of the Director of Client Services.
4. Makes recommendations to Client Services Supervisor for additions/modifications to client’s service plans.
5. Actively participates in crisis prevention and intervention for clients.
6. Provides training and assists in all aspects of vocational and life skills training, and activities.
7. Provides supportive supervision to client.
8. Monitors and relays information consistently regarding overall status of clients.
9. Assists with client’s personal cares and hygiene needs (feeding, toileting, dressing, dental hygiene, etc.).
10. Maintains safe, neat and orderly work area.
11. Specific, on-site training.
12. Facilitates job modification where applicable.
13. Arranges/provides transportation for client.
14. Initiates positive communication/relationships between clients and their co-workers/employers.
15. On-site evaluation of client’s performance.
16. May perform client’s job in the community for a short duration of time if client is unable or unwilling.
17. Assists in maintaining records and documentation.
18. Attends client service reviews, department, committee, and unit meetings as directed.
19. Assists with insite services as assigned.
20. Serves as a positive role model for clients.
21. Accepts other duties as assigned.
Qualifications
Associates degree in a related field. Will consider experience in lieu of a degree. Minimum of high school degree or equivalent (GED). Prior experience working with individuals with varying abilities highly desirable. Willingness to enroll in additional training as required. Must be flexible in the scheduling of work hours. Ability to perform the supported job in the absence of a client. Must possess a valid Wisconsin driver’s license and be insured in the state of Wisconsin. Must have reliable means of transportation.
