**Vanderbilt Home Care Services** **RN2 - $15K Sign On Bonus** **Job Summary:** The **Registered Nurse 2 with Vanderbilt Home Care Services**, applies the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning. As a Registered Nurse with the intermittent team, you will visit approximately 4 to 6 patients at their homes throughout your shift, providing the best patient care. The patients will be within 3-5 zip codes. Pediatric patients may be included. There is a mileage allowance with this position.The Registered Nurse with Home Care effectively communicates pertinent patient/family information to the health care team. Collaborates in establishing patient goals and evaluates progress to ensure effective outcomes. Furthers professional growth by engaging in learning activities according to the Board of Nursing standards.

**$15,000 Signing Bonus, mileage allowance. Vanderbilt Home Care-Intermittent Visit, for new, full time employees.**

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.

**KEY RESPONSIBILITIES:**

* Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.

* Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.

* Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.

* Peds experience or willing to work with Peds *(most patients are adult)*

**Licensure, Certification, and/or Registration (LCR):**

* Registered Nurse Required

* Heavy Work category requiring exertion of 50 to 100 lbs. of force occasionally and/or up to 20 to 50 lbs. of force frequently and/or up to 10 to 20 lbs. of force continually to move objects.
