Valor Healthcare is looking for a passionate Physical Therapist (PT) to join our team at the Community Based Outpatient Clinic (CBOC) in Houma, LA. Position Summary

The Staff Physical Therapist (PT) provides assessment and treatment intervention on an independent basis across the continuum of care and services provided by across the local VA healthcare system. Responsible for the examination and treatment of patients referred to the Physical Therapy Department. Provides in-service and clinical training programs as assigned by the Supervisor of the Physical Therapy Department or Rehab Coordinator.

Responsibilities

  • Maintains timely data input utilizing care plans, initial assessments and/or specialty protocols as part of an ongoing process in coordination with other key interdisciplinary staff to provide effective patient care.
  • Reviews and interprets Physicians orders for accuracy and appropriateness.
  • Reviews patient medical records and contacts physicians to make authorized changes required. 
  • Conducts comprehensive patient examinations to identify rehabilitation problems and establish treatment goals.
  • Evaluates patient response to treatment and modifies the treatment plan to provide optimum outcome.
  • The PT is capable of working with patients from a wide range of ages and backgrounds to include examination and treatment of patients in outpatient, inpatient, or home-based programs.
  • Demonstrates and maintains full competency in the use, pre-cleaning and/or reprocessing of reusable medical equipment. 
  • Demonstrates the teaching skills required to provide education to patients, family members, caregivers, employees, students, and coworkers.
  • Communicates effectively with other internal and external health care providers.
  • Performs competent written documentation of initial examinations, progress notes, and discharge summaries within the mandated timelines.
  • Participates in interdisciplinary care planning conferences.
  • The PT continually seeks to improve knowledge and skill by reviewing current literature, participation in in-service training, presentations in literature searches, and attendance at continuing education programs.
  • Competency in all general clinical functions but will also include coursework directed toward the development and improvement of clinical instruction, data collections and reporting techniques, and utilization and reporting of outcome measures.

Requirements

  • Education: graduate of a masters or doctorate degree program in Physical Therapy from a college or university accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) 
  • Licensure: current, full, active, and unrestricted license to practice Physical Therapy in Louisiana.
  • 2 years of experience ideally in an outpatient clinical setting.
