Valley's Best Hospice is Hiring a Nurse Practitioner, Per Diem Near Lancaster, CA

Under the supervision of VBH's Medical Director, provides direct patient care for patients under Hospice and Palliative Care services. Provide patient/unit of care education regarding chronic health conditions, Advance Directives, palliation, disease progression and end of life symptom management.

Essential Duties and Responsibilities include the following:

  • Patient visits in the patient's residence (home, Residential Care Facility for the Elderly, or Skilled Nursing Facility) for the purposes of symptom management and assessment of other chronic illnesses at the request of the patient, family, or a member of the interdisciplinary team. Visits to meet regulatory compliance with recertification prior to applicable benefit period (face-to- face) and Skilled Nursing Facility compliance. Assessment of conditions related to the terminal condition if discharge for extended prognosis is Intervention for potential revocation of hospice services.
  • Collaborate and participate with Hospice Physician, RN Case Manager & Interdisciplinary Team in development and review of the comprehensive plan of care.
  • Participate in development of Palliative Care Program to meet the health care needs of identified population.
  • Participate in development of Hospice Program to meet the health care needs of identified population. Participates in Quality Assessment Performance Improvement in collaboration with Quality Manager: Auditing clinical records for regulatory compliance. Case Review to help identify performance improvement opportunities and subsequent educational materials. Preparation for regulatory surveys and medical reviews.
  • Participates in educational activities related to performance improvement activities and regulatory changes: Preparation for regulatory surveys and medical reviews. Staff orientation and continuing education. Facility education and outreach. Provider education and outreach.
  • All other duties as directed by management.

Education and / or Experience:

  • Advance Degree Nursing/Advance Practice Nurse Certification and Furnishing/DEA.
  • Current valid California Nurse Practitioner License.
  • Five years of healthcare experience. Hospice experience preferred. Prior experience in a managerial or supervisory capacity. Program development experience is desirable.
  • Preferred experience in managed care environment/or a clear understanding of managed care systems.
  • BLS required within 60 days of hire.
  • Is self directed and must have excellent organizational and managerial skills.
  • Excellent interpersonal communication (written and verbal) skills.
  • Driving required. Current California driver's license and proof of insurance.
  • Experience with Quality Assessment Performance Improvement/Medicare Conditions participation.

The pay range for this position at commencement of employment is expected to be between $60.10 - $62.50 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
