The pharmacy technician will perform medication histories/reconciliation through patient/family/significant other interviews for patients arriving in the Emergency Department to be admitted to the inpatient units. The role requires an appropriate level of understanding and training in medical terminology and medication related terms. Responsible to maintain confidential reports, enter and retrieve data utilizing the computer system, and speak confidently with patients/family, doctors, nurses, and other healthcare professional

Essential Functions:

• Performs medication history collection and reconciliation for Emergency Department patients
• Interviews patients or patients' family to verify accuracy of list
• Calls patients' pharmacies to verify accuracy of medications when necessary
• Verifies or adds patients' preferred pharmacy
• Verifies or adds patients' allergy information including reaction and severity of reaction
• Oversees the medication distribution process for the Emergency Department
• Manages inventory and par levels of Emergency Department automated dispensing units
• Replaces medications (including control drugs) to automated dispensing units during pharmacy operating hours
• Informs charge nurse/ clinical coordinator as well as pharmacist of all control drug inventory discrepancies assuring all discrepancies are resolved within 48 hours
• Actively participates in quality improvement activities
• Collects and analyzes data accurately for projects and initiatives specific to the Emergency Department
• Completes and documents medication storage area inspections on a monthly basis for the Emergency Department
• Identifies and replaces outdated and unusable drugs
• Prepares sterile product according to approved procedures, documentation requirements and aseptic technique

Qualifications:

• High School diploma or the equivalent.
• 3 years as Pharmacy Tech – preferred
• NH Pharmacy Technician registration within 14 days of hire
• National Pharmacy Technician Certification from PTCB, NHN within 90 days of hire
• High integrity, able to follow procedures
• Ability to be a self-motivated learner, to take initiative in learning the many complex processes
• Exhibits professionalism at all times, understands appropriate workplace behavior/communications.
