Valley Oaks Health, a local community mental health center located in Lafayette, Indiana, is searching for applicants to join our ever growing team. Are you passionate about making a positive impact on the lives of children and families facing mental health and emotional challenges? Join our team as an Intensive Care Facilitator!
As an Intensive Care Facilitator, you will collaborate closely with children and their families to address and manage their mental health and emotional difficulties. The primary focus of your role will be to stabilize and improve social and behavioral functioning within the home, school, and school environments.
Responsibilities:
- Work directly with children and families to develop personalized support plans aimed at managing mental health and emotional difficulties.
- Provide intensive case management services to ensure the stabilization and improvement of social and behavioral functioning.
- Assist clients in building personal support systems and empowering them to advocate for their needs effectively.
- Implement treatment plans focusing on the development of problem-solving and coping skills tailored to the unique needs of each child and family.
- Offer guidance and support in utilizing appropriate conflict resolution strategies.
- Provide timely emergency services and crisis intervention in various settings, including homes, hospitals/ERs, and the community.

Who We Need:
Bachelor's Degree in Social Work, Psychology, or other human behavioral science field. Degree programs with related volunteer and/or work experience will also be considered
Must have a valid Indiana Driver’s License, a reliable vehicle, a good driving record, and car insurance
Ability to work flexible hours including evenings and weekends as needed
Good usage of the English language; reading, writing, and speaking required.
High level of interpersonal skills to handle sensitive and confidential situations and documentation
Attention to detail in composing, typing and proofing materials, establishing priorities and meeting deadlines
Excellent spelling, grammar, and written communication skills
Excellent telephone and oral communication skills
Ability to maintain a high level of confidentiality
