Description

NATURE OF WORK:

The Activities Facilitator assists in the operation and maintenance of the agency challenge course.

The Activities Facilitator assists in providing recreation opportunities to Valley HealthCare System clients. The activities are but not limited to bowling, fishing, swimming, hiking, pottery/arts and crafts, etc. Activities Facilitator must be engaged with the clients with positive enthusiasm for the activity to be a leader and facilitator. Activities Facilitator must be able to work with all population types Substance Abuse, Mental Health, and IDD.

This position also involves some minimal work in Valleys Ropes Course. This includes the setup, facilitation and processing low ropes initiative games. Other duties include set up of the high ropes course, which involves moving, and setting out necessary equipment, climbing up a ladder, attaching climbing holds, and setup of belay ropes. In addition, the Activities Facilitator will assist in the ground school and help teach clients the proper safety protocols while on the high ropes. The Activities Facilitator will also belay and assist in the transfer of clients on the ropes course ensuring all safety protocols are followed.

ESSENTIAL DUTIES:

  • Climb to heights of 45 feet
  • Conducts daily and monthly inspections of the ropes course
  • Setup and take down the ropes course
  • Facilitating general recreational activities, including bowling, fishing, hiking, etc.
  • General knowledge of how to run recreational groups, including general recreational procedures, how to process recreational activities in a manner that is beneficial, and how to link the activity to a clients treatment plan to demonstrate medical necessity
  • Tie knots, belay, and work with rope, harnesses and other safety equipment
  • Assist with light maintenance and general upkeep of the course
  • Assist with the completion of documentation including rope logs
  • Lead and assist with low program initiatives
  • Facilitate processing using experiential therapy techniques with both the low and high elements
  • Recognize, assess, and manage safety aspects of all elements and participants
  • Lead and assist with the ground school instruction
  • Uses clear communication skills, including person first language with clients and other professionals
  • Attend trainings as required
  • Complete documentation as required by Valley, Medicaid, OSHA, and other licensing agencies. Employee ensures that documentation (such as Direct Services Progress Notes, Transportation Notes, Task Analysis Forms, Behavior Tracking Forms, etc.) are completed via the Electronic Medical Record system accurately and timely and will reflect services the client(s) has received services as appropriate.
  • Complete Comprehensive Community Support Documentation during each daily episode of care, notating, clinically, clients progression/regression in each of the clients individualized treatment goals identified in Master Treatment Plan
  • Facilitate and lead groups of people of no more than 1:12 ratio.
  • Completes and submits program specific documentation in a timely and accurate manner.
  • Transportation of consumers to appointments/activities in the community using a company vehicle.
  • Support and help in the training of new employees.
  • OTHER DUTIES AS ASSIGNED.

Qualifications


MINIMUM QUALIFICATIONS:

  • Physical agility required to participate in the high and low challenge course initiatives
  • Valid Drivers License
  • Ability to comply with Clients Rights
  • Ability to comply with Valleys and Division departmental safety procedures
  • Ability to read, write, understand and speak the English language
  • Current CPR/First Aid Certification or ability to be trained and maintain certification
  • Current MANDT or behavior de-escalation techniques and ability to maintain certification
  • Ability to perform Essential Duties as outlined below

PREFERRED QUALIFICATIONS:

  • One year experience in a similar position
  • Previous experience working in behavioral health
