Job Overview:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and managing the care of patients, ensuring they receive the highest quality of care and support throughout their healthcare journey. We are recruiting in New York and Nationwide.
Responsibilities:
- Conduct comprehensive assessments of patients' medical history, current condition, and healthcare needs
- Develop and implement individualized care plans in collaboration with the patient, their family, and healthcare team
- Coordinate and facilitate communication between healthcare providers, specialists, and other stakeholders involved in the patient's care
- Monitor and evaluate patients' progress towards their healthcare goals
- Provide education and support to patients and their families regarding their medical condition, treatment options, and self-care techniques
- Collaborate with social workers, discharge planners, and insurance companies to ensure appropriate discharge planning and utilization of resources
- Document all patient interactions, assessments, interventions, and outcomes accurately and timely
Experience:
- Valid Registered Nurse (RN) license in [state]
