VA Northern California Health Care System is Hiring a LVN - Mental Health - Chico Near Chico, CA

The Licensed Vocational Nurse (LVN) functions under the direction of a Registered Nurse (RN) or MD. The incumbent performs a wide range of patient care and duties throughout NCHCS. The incumbent has the knowledge and skills to carry out nursing assignments for ambulatory care and psychiatric nursing care. The LVN is responsible for observing, recording, and reporting changes in the patient’s condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LVN is a member of the expanded team she/he requires a general review of their work by an RN for appropriateness and conformity with license, policies, and procedures.

Major duties include, but may not be limited to:

➢ Chart effectiveness or refusal of psychotropic medications in CPRS. Notify RN and/or medical provider of medication effect or refusals.

➢ Perform duties as assigned by the nurse manager, clinic RN, and program manager.

➢ Notify RN and/or medical provider of incorrect or duplicate orders. Flag incorrect orders, and send them to provider, if not emergent.

➢ Assist with coordination of admission of patients to an acute mental health inpatient unit, as able.

➢ Complete breathalyzer, glucometer, urine tox screens, as ordered. Administer medication, per order. Use VA protocol for hypoglycemia. Notify RN and/or provider of abnormal, out-of-range, results.

➢ Assist with VS, clinical reminders (Alcohol, Depression, PTSD, Tobacco, PCL, TBI, OEF/OIF, MST), and other care needed prior to seeing a clinician, and report abnormal results to RN or medical provider. ➢ Monitor on-site medication inventories, injections, and AEDs; ensure supplies stocked and medications appropriately counted.

➢ Provides direct nursing care to groups of patients for the Mental Health Outpatient Clinic.

➢ Works under the direction of the RN/MD but day-to-day operations is responsible to the Program Manager.

➢ Serves as a member of the mental health care team providing care on a regular and recurring basis.

➢ Utilizes knowledge and assists BHIP prescribers in patient care problem making appropriate referrals to the RN, including the ability to identify emergent needs.

➢ Administers medications and collects specimens as needed.

➢ Is familiar with and performs/meets Clinical Practice Guideline and Prevention Index standards within his/her scope of practice.

➢ The incumbent must adhere to all safety standards, policies, and procedures.

➢ Able to work with excel documents and able to review medical record as well input data in the electronic dashboards, collects patient medication information and inputs changes in the computerized medical record.

➢ Documentation of patient care is done electronic medical record.

➢ LVN will assist with patient clinical reminders prior to their GMH appointment.

➢ LVN will be making various phone calls to Veterans, either to remind them of future appointment,

pre-appointment screening, and creating and sending reminders letter within the LVN Scope of practice.

➢ Alert provider by being made an additional signer and/or by other established process to any positive screens

➢ LVNs should document vitals only on patients who require them (e.g., antipsychotic monitoring which requires blood pressure and weight) and on any patients whom the providers request vitals on.

➢ LVN will assist with calling high-risk patients the day prior to a scheduled appointment to ensure these patients do not miss appointments.

➢ LVNs should administer decanoate injections at the direction of the provider. The LVN is responsible for interfacing with pharmacy to obtain the medication after the order is written and for coordinating injection appointments with the patient.

➢ LVNs should administer flu shots at the direction of the provider.

➢ LVN will carry out all other duties as assigned.

Pay: $50,613.00 - $92,119.00 per year
