Speech Language Pathologist (SLP)

The University of Vermont Health Network Home Health & Hospice is looking for a Speech Language Pathologist who will provide a full range of speech language pathology services to clients in the home setting, under general clinical supervision. The Speech Language Pathologist will perform and participate in speech language pathology education to staff, clients, and others. Other responsibilities include:
Implementing treatment plans to regain maximum independence, completing ongoing assessments of current status, teaching clients about their conditions and ST treatment plans, and counseling clients regarding psycho-social problems related to speech and language disabilities
Planning and coordinating a variety of services for clients provided by The University of Vermont Health Network Home Health & Hospice and other community health care and human service organizations, while performing a variety of patient advocacy functions
Conferring with patient's families, physicians, and other care providers to coordinate services, discuss patient's status, needs and problems
Preparingall required clinical and administrative documentation related to assigned activities, including chart notes, documentation of conversations, and care plans
Qualifications:
Masters Degree in Speech Pathology
Three of more years of speech language pathology experience with home care experience preferred
Ability to travel in order to conduct home visits
