Responsibilities

  • Develop diabetes self-management education and pre-diabetes related education resources for patients referred for consultations. This includes patient assessment and education on diabetes self-management
  • Provides education and training for care managers, medical assistants and nursing staff regarding diabetes self-management using the 3 M of self-care and other care plans/resources in alignment with ADA guidelines.
  • Assess clients’ nutritional, health needs and social determinant needs
  • Conduct in person, Televideo and groups education and counseling
  • Participates in community events on the topic of healthy eating and diabetes education.
  • Documentation care plans in Epic
  • Evaluate if the meal plans have a positive effect on clients’ health
  • Create educational material about healthy eating habits
  • Speak to groups about the importance of nutrition to prevent specific diseases
  • Document patient progress

Qualifications

Job Requirements:

  • Certification as a Diabetes Educator by the American Diabetes Association
  • Registered dietitian in the state of OklahomaOR
  • Registered nurse in the state of Oklahoma

Preferred Job Requirements:

  • 3 years of experience as a dietitian or RN
