We have 4 primary care health centers s and deliver integrated services including behavioral health, Pharmacy, Dental, specialty referrals, chronic disease management, health education, and much, much more. UNHS is hiring a Full-Time (FT) position for Gentle Iron Hawk Shelter Therapist.
  • Locations: Blanding, UT

Critical Tasks

  • Provide clinical services to adults and children who may be suffering with a mental illness, addiction and trauma reaction.
  • Collects data using observation, interviews and counseling sessions to specifically diagnose and assess behaviors.
  • Works closely with Navajo Traditional Counselors to provide and support culturally competent services .
  • Works in conjunction with other behavioral health professionals such as Nurse Practitioners and Psychiatrists to derive an appropriate treatment plan.
  • Provides interventions and therapy by identifying behaviors and prescribing effective activities for behavior modification.
  • Provides services to victims of domestic violence, and sexual assault.
  • Collaborates with families to keep them informed and to understand client feelings and responses to various treatment plans .
  • Uses a variety of skills such as counseling, communication, clinical, problem-solving and interpersonal skills to monitor and manage prescribed treatment plans.
  • Maintains detailed client/clinical records into the EHR System and updates as required to assure behavioral modification goals are achieved.
  • Instructs clients on how to use the necessary skills and exercises to build social, transitional, language, cognitive and interactive skill development need for behavioral change.
  • Participate in Quarterly Chart Audits.
  • Hypothesizes situations and conducts situational analyses to assist clients with problem solving activities needed for behavioral change.
  • Counsels and provides interventions and works with other practitioners such as Nurse Practitioners and Psychiatrists to develop treatment plans and interventions for SMI (seriously mental ill) clients with borderline personality disorders, suicidal thoughts and self-harming behaviors.
  • Utilizes group and individual therapy to engage clients in reconstructing non-productive behaviors and acquiring behavioral skills that will remove harm and diminish negative psycho-social behavior.
  • Provide crisis intervention, suicide prevention, information & referral, and brief supportive counseling to patients who are in emotional distress and/or seeking information on available mental health services.
  • Provide follow-up to high risk callers reporting suicide and/or homicidal thoughts and behaviors.
  • Attend and participate in Clinical Supervision and Behavioral Health Department Meetings as required by Department Leader.
  • Provide crisis intervention, suicide intervention, safety planning, and responsible for assessment and determining level of care for patients.
  • Collaborate with public health or other departments for outreach or educational services in the community.
  • Collaborate with elementary, middle schools, high schools, and colleges within our service area for outreach services to students and their families.
  • Provide crisis counseling services for individuals, families as in person or telehealth service and determine resources and referrals for higher level of care needs.
  • Provide Covid-19 response efforts to support public health needs on educating patients of protective measures at home and in the community, providing PHI to patients on test results, support services, answering questions for patients, and also preparing care packages/resource materials and supplies to be delivered to patients in quarantine or isolation status.
  • Provide therapeutic intervention skills to children, youth, adults, and geriatric populations.
  • Provide debriefing patients or staff on traumatic experiences or situations.
  • Provide therapy and behavioral health coordination to the students and Utah State University at the Blanding campus
  • Provide services at any UNHS location as determined by Department Director

Requirements

  • Inputs client data in the EHR (electronic health records) or current record-keeping system
  • Collaborates with other licensed practitioners effectively to finalize, change or develop new clinical interventions
  • Monitors client intake of prescribed medications and reports any noticeable behavioral changes to Prescribers
  • Communicates effectively with clients, client families, clinical staff and other prescribers involved in the overall treatment plan
  • Utilizes current behavioral health techniques in the cognitive therapy work with adults and children
  • Demonstrates compassion in communications, active listening, patience and interpersonal skills
  • Demonstrates sensitivity and an appreciation of the culture of the Navajo People
  • Shows an adherence to policy, procedures of the Utah Navajo Health Clinics and understands the regulatory laws governing behavioral health practitioners in the State of Utah
  • Utilizes certified training(s) to help with clinical needs of organization, with supervisor approval of added responsibilities.
  • Participation in grant writing and research data collection.
  • Provides “SAFETY PLANNING” for suicidal patients
  • Outreach to grieving families from the loss by suicide.

Metrics

  • Maintains appropriate licensure; licensed counselor or licensed social worker
  • Maintains ongoing CEU’s (continuing education credits) as required
  • Meets caseload requirements as prescribed by the agency
  • Provides appropriate reporting as required by the Chief Medical Officer for UNHS
  • Meets all auditing requirements for regulatory oversight
  • ASIST certified within 3 years at any given time (Applied Suicide Intervention Skills)
  • Competency to provide Post Vention Skills to families who have lost a loved one to suicide.

Qualifications

  • Master’s degree in Social Work or related field. E.g. Master’s degree in Professional Counseling, Mental Health Counseling, Substance Abuse Counseling, or specialized therapy skills such as Trauma
  • Licensed or Associate licensed professional in the State of Utah e.g LCSW, LPC, LCMHC, CMHC-I, ACMHC, etc.
  • Exceptions to Master’s level professionals with 5 years experience in completing assessments, group counseling, or certifications specializing in any mental health or substance abuse related counseling.
  • Navajo preference
  • Prefer experience working with Native American communities

As a Tribal Organization the Utah Navajo Health System, Inc. (UNHS) treats patients with high risk and underlying chronic medical conditions. Therefore, UNHS requires its employees to show proof of immunization prior to their employment with UNHS or during their employment. Below is a list of immunizations/vaccinations required to be employed or to continue employment with UNHS.

The immunizations/vaccinations include but not limited to following:

  • MMR (Measles Rubeola, Mumps, Rubella): Documentation of two MMR vaccines (OR) Documentation of MMR titers.
  • Complete Hepatitis B vaccine series (3 doses), documented proof of titers indicating immunity, or a declination that may be signed upon arrival.
  • Proof of up-to-date PPD skin testing, if previously negative (OR) Proof of positive PPD skin test with the most recent chest x-ray and treatment history.
  • Tetanus, diphtheria, and pertussis (Tdap).
  • Varicella titers, history of varicella or varicella immunization series.
  • Influenza vaccine for the current year.
  • COVID-19 vaccine
  • Any other vaccinations as requested.
