Job Overview: As our growth continues, Utah Home Health and Hospice is seeking a Weekend Visit Registered Nurse/RN (Home Health/Hospice). Your primary responsibility would be to assist in PRE-PLANNED visits on Saturday and Sunday. There will be an on call nurse to help with needs that are anticipated.

Responsibilities

• Monitor patient’s condition and assess their needs to provide the best possible care and advice
• Perform patient care as required, including home assessments.
• Ensure and chart compliance with state regulations.
• Evaluate and treat patients using the most current technology and practices.
• Provide valuable input into the patient's care and treatment.

Qualifications:

• Licensed as a Registered Nurse by the State of Utah
• Experience in Home Health or Hospice preferred
• Experience with Homecare Homebase or other EMR not required but a HUGE plus
• Ability to communicate effectively, both verbally and in writing
• Respecting the patient’s privacy and maintaining confidentiality
• Current Utah driver’s license and automobile liability insurance
• CPR Certification
