Job Overview:

Utah Home Health and Hospice is looking for a dynamic, motivated, and compassionate Weekend Visit Registered Nurse/RN (Home Health/Hospice) to join our patient -centered care team. The primary responsibility of the home health and hospice Weekend Visit Nurse is to provide direct support to the patient, their families, and the Oncall Nurse with scheduled weekend visits while functioning within a medically approved plan of care. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be compassionate and cool-tempered.

Primary Responsibilities:

* Work as part of an interdisciplinary team that includes physicians, nurses, social workers, chaplains, massage therapists, certified nurses aides, etc.*

• Perform weekend visits as needed by the organization.
• Work with primary care physicians and the hospice medical director.
• Accept and understand the mission of our home health and hospice.
• Ensure that the educational needs of the patient and family are met.
• Review and document patient needs in care plans, progress notes, etc.
• Monitor patient’s condition and assess their needs to provide the best possible care and advice
• Administer medications and change wound dressings.
• Provide instant medical care in emergencies.
• Travel Required

Qualifications:

• Must possess current, unencumbered, active license to practice as a Registered Nurse.
• Must have valid care insurance and driver's license.
• Knowledge of and experience in pain and symptom management.
• Well-developed physical assessment skills.
• Must be able to read, write, speak, and understand the English Language.
• Must possess patience, tact and a positive attitude.
• Hospice/Home Health experience preferred.
• Must have valid CPR.
