As an Admissions Nurse at Aspen Physician Group, you are an essential part of delivering proper health care. Aspen Physician Group is a thriving physician owned and managed primary and specialty care practice catering to adult and geriatric patients across the state overseeing 100's of patients. We see our patients in assisted living, independent living, nursing homes and their private homes. Each clinician has a remote support team to assist with daily tasks (medical assistant/office support). Out in the field, you will work closely with Assisted Living, Home Health and Hospice staff in ensuring the health and safety of our patients. Job Description:

• Come in the office at 9am (if there is a 9am admission scheduled)
• Work on charting and QA audits.
• Assess, supervise and direct patient care.
• Observe and record patient behavior.
• Positive attitude and desire to work with the aging population.
• Perform patient care as required, including home assessments.
• Collect patient health history.
• Develop plans of treatment.
• Ensure and chart compliance with state regulations.
• Evaluate and treat patients using the most current technology and practices.
• Provide valuable input into the patient's care and treatment.
• Administer medication, change wound dressings and care for other treatment options.
• Read and interpret patient information, making treatment decisions when appropriate.

Job Requirements:

• Licensed as a Registered Nurse by the State of Utah
• Current Utah driver’s license and automobile liability insurance
• BLS Certification
• Minimum 2-3 years of Hospice Experience
• Ability to communicate effectively, both verbally and in writing
• Respecting the patient’s privacy and maintaining confidentiality
• Experience with Homecare Homebase or other EMR not required but a HUGE plus
