USF Tampa General Physicians Department of Pediatrics is Hiring a RN - Complex Care - Pediatrics Near Tampa, FL

POSITION SUMMARY:

This Registered Nurse (RN) will provide case management and patient care services to patients for the Pediatric Complex Care Clinic under the General Pediatrics Division of USF. As a member of the Pediatric Complex Clinic team they will be helping to care for children with chronic or complex medical issues. The RN be responsible for coordination services such as appointment scheduling and medical record management in addition to clinical services such as patient education and bedside care/procedures. This position requires providing information to patients in person and over the phone. As such, a bilingual English and Spanish skill set is preferred. Traveling to various practice site locations is required.

All employees who will work with the primary care department will participate in a patient centered medical home that orients primary care towards a holistic picture of wellness.

Employees need to approach care as a team and help patients coordinate across all elements of the healthcare system. They must convey a positive image of USF Tampa General Physicians and USF Health.

This position will report to the Complex Clinic Director, Assistant Director, and Supervisor Nurse. Additionally, this position will adhere to the Department of Pediatrics policies and procedures, including but not limited to completing required online trainings, attending customer focus training, and following dress code policy.

Note: Must successfully demonstrate competencies within job position.

SPECIFIC DUTIES AND RESPONSIBILITIES:

40% Clinical Duties:

· Serve as a member of the multidisciplinary team by attending various clinics with providers.

· Provide direct hand-on patient care including, but is not limited to, assisting provider with patient care, medication administration (including oral, inhaled, and injectable medication), assisting with adaptive technology (including feeding pumps, gastric tubes, tracheostomies).

· Provide patient education and anticipatory guidance

· Create and deliver patient care plans to families

· Act as a liaison between providers, patients, pharmacies, hospitals and non clinical staff to provide a team approach to patient care

· Act as patient advocate to ensure all patient concerns are address in a timely and professional manner

40% Administrative Duties:

· Schedule patient appointments

· Coordinate referrals to subspecialty services

· Coordinate patient Home Health paperwork and records

· Coordinate paper work and orders for patient adaptive technology, home equipment and supplies

· Monitor and follow up on patient no show appointments to reschedule as needed

· Assure patient messages are reviewed as they arrive, delegate as indicated follow up in a timely manner, with all actions documented in the electronic health record.

· Attend meetings and participate in clinical committees and task forces as assigned or deemed necessary

· Assist in managing disaster planning protocol including enrollment, patient communication, and collaboration with team

· Report in timely manner to Complex Care Clinic Charge Nurse

15% Quality Improvement/Data Collection:

· Assess quality of care and assist in identifying opportunities to improve patient services.

· Collaborate with clinic manager/supervisor to implement process and system changes

· Work with provider leadership in assigned clinical areas to establish written protocols to increase quality and efficiency of patient messaging, tasks, procedures, and other patient care areas.

· Assist in collecting data for benchmarking and compliance reporting

· Assist in keeping patient lists organized and up to date

· Serve as a resource for HIPAA and Patient Safety policies and standards of care

· Assist in outpatient data collection as needed

5% Other duties as assigned

POSITION REQUIREMENTS:

Education: Graduation from an accredited school of nursing with a current state of Florida RN license.

Experience: Minimum 1 year nursing experience in a pediatric healthcare setting. Clinic experience in an ambulatory healthcare facility preferred. BLS/AED certification required and must be maintained. ACLS or PALS certification preferred.

Business Skills: Excellent patient assessment skills, strong communication and interpersonal skills, good logical and reasoning skills, excellent organizational skills needed. Experience and competence with electronic medical record systems required. Proficiency in computer skills including MS Office required.

Physical Demands: Must have the ability to lift 20 pounds. Moving, lifting or transferring of patients may involve lifting up to 50 pounds, as well and assist with weights of more than 100 pounds. Must have ability to stand for extended periods, fine motor skills, visual acuity. The employee will occasionally need to kneel or crouch. The employee will be frequently required to walk, use arms, hands and fingers to handle, feel and reach forward to grab. While preforming

the duties of this job, the employee will be regularly required to sit, stoop, stand and talk or hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust or focus.
