Provide medical case management services to patients and families with advanced or chronic illness; act as a liaison with other community resources; complete required documentation on patients, including program eligibility determination, psychosocial assessments with care plans, enrollment in health insurance programs, and medication assistance programs; maintain established departmental and grogram-specific policies/procedures, objectives, safety, environmental and infection control standards, and quality improvement; provide case monitoring and obtain updated patient contact information during medical appointments; enter case management services into program data base; document case management notes in HER; submit monthly case management report to program coordinator; assist in development of patient education programs; enhance professional growth and development through participation in education programs; identify non-medical patient/family needs for other support services, such as housing, legal issues, and assistance with transportation, utility bills, and food; provide individual and group counseling to patients and families; present educational programs to the community and/or hospital/clinic staff; participate in monthly staff and quarterly quality management meetings; regular and prompt attendance; ability to work schedule as defined and overtime as required; related duties as required.

Masters degree in social work from an accredited institution as approved and accepted by the University of South Alabama; or Bachelors degree in social work from an accredited institution as approved and accepted by the University of South Alabama and two years social work experience; or Bachelors degree in a related field a from an accredited institution as approved and accepted by the University of South Alabama and three years social work experience. Licensure with the State of Alabama as a Social Worker.
