Responsibilities

Serves as a resource and liaison to the Transfer Center Specialists, Transfer Center Specialist Supervisor, Transfer Center Coordinators, nursing departments, and hospital administrators while utilizing professional communication and conduct that illustrates quality leadership skills and respect to others; Oversees the coordination of incoming transfers and acceptance of patients for USA hospitals through planning, evaluation, and communicating with all needed service lines, other nursing department staff, Transfer Center staff, and referring facilities while being able to state the purpose, rationale, and appropriateness of any given referral ensuring hospital policies are being followed; Abides by, supervises, and enforces all compliance requirements, departmental policies, and performs these responsibilities in an ethical and professional manner consistent with the organization's values; Adheres to hospital policies including confidentiality; Requires regular and prompt attendance; ability to work schedule as defined and overtime as required to include aiding in covering vacancies in the Transfer Center including coordinators and Transfer Center Specialists; related duties as required; Serves as preceptor for new employees, assists in maintaining employee files to include ensuring staff have active licenses for nurses and paramedics including ACLS, PALS, BLS, stroke hours, education, and performing staff evaluations; Requires minimal supervision to safely perform departmental duties to include supervising the following: managing throughput, diversion, following up on discharges and delayed internal transfers, scheduling, payroll, data entry, data analysis, responding to emergency bed requests, maintaining unit supplies, problem and compliant resolution, and the escalation of issues through the established chain of command in a timely manner; Collects clinical information for a transfer request and facilitates the transfer by matching the patient’s needs to the facility with bed capacity and service capability and ensures accurate and concurrent data collection and maintenance; Utilizes a personal computer or terminal to access the Hospital Information System; Communicate, collaborate, and use appropriate customer relation skills with physicians, referring facilities, administrators, and healthcare team in person and via telephone to foster relationships with referring facilities, physicians, and hospital staff; Works closely with the Transfer Center team and senior leadership to identify trends and report barriers to patient throughput, delayed discharges, and diversion; Uses problem-solving skills to determine the course of action to generate the best patient clinical outcome, the physicians’ preferences, and the transferring and receiving hospitals; Aide and oversee staff in following up with referring facilities who are on the waiting list with a referral request once diversion status allows the acceptance of inpatient referrals; Coordinates transfer of emergent, urgent and elective patients to a final destination facility from all potential referral sources to include but not limited to hospitals, clinics, nursing homes, and physician offices via a single call to initiate acceptance and expedite decision making relative to a potential transfer; Performs and oversees transfer call center functions in a responsive manner answering calls, entering into a database consisting of facility service profiles, approved clinical protocols, established patient care protocols, and healthcare information; Facilitates and supervises patient throughput to a final destination with an ongoing focus on quality, efficiency, and the reduction of diversion hours; Performs and oversees the triage and prioritization of patient information and communicates an accurate clinical presentation of the patient for accepting physicians and nurses; Completes all training and is competent in the electronic applications related to job functions; Adheres to, supervises, and enforces regulatory requirements such as EMTALA, Ethics and Compliance policies, and quality initiatives; Monitors compliance and aids senior leadership in implementing process changes to ensure compliance to such regulations and quality initiatives; Supervises the communication with physicians, case management, and outlying facilities regarding all patient transfers, potential bed placements, and assists with coordinating patient movement from one facility to another; Accepts and completes all duties positively and without conflict while maintaining a positive and structured work environment through teamwork, cooperation, and effective communication; Assist nurse manager with all of the responsibilities under their command to include: shift assignments, attending meetings, written disciplinary actions, evaluating personnel performance on a continuous basis, budget control, staffing office needs, payroll, data entry, data collection, data analysis, patient throughput management, reduction of diversion hours, maintaining unit supplies, scheduling, and all other duties as assigned with all departments under the command of the Transfer Center nurse manager.

Qualifications

Graduation from an accredited school of nursing and a current Alabama RN license or current Multi-State RN License in accordance with the Nurse Licensure Compact (NLC) for Alabama. Note: USA Health abides by the NLC requirements and guidelines for the state of Alabama.
