The Home-Based Primary Care Program (HBPC) is a multi-discipline primary care program that provides longitudinal and comprehensive primary care for persons with complex chronic diseases for whom traditional clinic-based care is not effective or may not be possible. HBPC provides cost-effective and patient-centered primary care services in the home setting with the goals of improving chronic disease management, decreasing hospitalizations, providing support for caregivers, allowing Veterans to be safely cared for in the home setting, delaying or preventing nursing home placement, and assisting with transitions of care.
The HBPC Nurse Practitioner is a full-practice authority provider that provides primary care for a panel of patients within the program under the supervision of the HBPC Medical Director and direction of the HBPC Program Director. This position is located at the Richmond VAMC, Richmond, Virginia.
The HBPC Nurse Practitioner must be able to perform the following:
Preferred Experience: 3 years of Nurse Practitioner Experience in Geriatric Primary Care, Adult Primary Care, House-calls/ 2 years Home Based Primary Care.

Physical Requirements
The physical requirements of this position includes lifting, prolonged standing, frequent bending, stooping, walking, reaching, repetitive motion, climbing stairs, and driving in a car for up to two (2) hours in one direction. The mental/sensory requirements include recall, reasoning, complex problem solving, hearing, clear distinct speech, adequate comprehension, writing, keyboarding, reading, logical thinking, and using electronic medical records at the facility. The environment's pace can vary from steady to a sometimes-fast pace requiring handling of multiple priorities in a single day, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes-noisy environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness, and hazardous materials. This position involved working with patients that have communication difficulties, cognitive loss, altered mentation and caregivers that are stressed. Collaboration with multiple stakeholders is essential. This position requires compliance with VHA mandated vaccinations and meet the employee health requirements.
