Description

FUNCTION: As the Liaison of Family Care and Incident Management you will be responsible for overseeing the Family Care Program and assist in reporting and investigating all Incidents that occur with individuals receiving services in the Developmental Disabilities Department. As the Liaison of the Family Care Program you will be responsible for, but not limited to, the following: provider and client recruitment, intakes, home inspections/safety plans, home visits, provider trainings, maintaining client and provider case records, creating, reviewing, and updating the Residential Habilitation plan and working with care coordinators in Life-Plan meetings for the Family Care program. You will provide guidance to providers and clients enrolled in the program, ensuring client safety and supporting Life-Plan outcomes. You will be responsible for working effectively with the Family Care providers, clients, internal and external service providers to ensure the program carries out the policies, procedures and responsibilities of the individuals who will participate in the program, following OPWDD guidelines. In coordination of all reported incidents related to program Individuals you will be responsible for, but not limited to, the following: ensuring the onboarding and annual incident training for all applicable staff and other program participants (e.g., family care providers), making appropriate notifications within the required timeframe and reporting, documenting, and following-up on all reportable incidents. Ensure the monthly review of all incidents, appropriate closure, and documents are maintained in accordance with OPWDD guidelines. 

RESPONSIBILITIES:

Family Care Program

  • Responsibility to Enrolled Consumers and Consumer Record:
  • Develop Family Care Residential Habilitation goals in conjunction with Life Plans with input from the participant, family, advocate, care coordinator, and other staff.
  • Life Plan input is provided as requested by the program participant and Care Coordinator.
  • Recommends Residential Habilitation Services that are designed to meet the participant’s choice.
  • Is prepared for all meetings, having reviewed all pertinent information and demonstrates a good knowledge of the person’s Life Plan, Residential Habilitation Plan, interests, assets, obstacles and development towards achieving life goals and /or valued outcomes.
  • Remains in compliance with applicable regulations, and particularly with the elements of independence, inclusion, individualization, productivity, and governing principles of OPWDD.
  • Covers case review meetings as requested and provides input relevant to the program services being provided.
  • When unable to attend case reviews, ensures coverage by someone else in the program area familiar with the program participant. Submits written input form to program administration if no one from the core area is able to attend.
  • Provides ongoing encouragement to consumers toward achieving goals of independence. 
  • Attention is given to assure consumer’s rights are addressed and respected. 
  • Communicates between service providers for all significant occurrences/events. 
  • Completes or updates consumer’s Individual Plan of Protection Family Care Addendum on an annual basis and as changes occur.
  • Completes all required documentation, including ensuring that progress notes are completed and filed in the person’s Family Care record.
  • Maintain monthly summaries in accordance with billing requirements.
  • Completes accurate, timely program billing.
  • Ensure participant records are maintained and updated in accordance with OPWDD compliance regulations. 
  • Track and document all compliance issues and develop and implement mitigation strategies to reduce risk. 
  • Responsibility to Family Care Provider and Family Care Home Record:
  • Provides supervision and direction to support Family Care providers in carrying out role, responsibilities to the program participant and OPWDD guidelines. 
  • Ensure providers receive the necessary onboarding, annual, and as needed trainings in accordance with OPWDD guidelines and agency requirements.
  • Conducts monthly home safety inspections, quarterly fire drills, and annual home certifications. 
  • Ensure provider records are updated in accordance with OPWDD compliance regulations.
  • Track and document all compliance issues and develop and implement mitigation strategies to reduce risk. 

Incident Coordination

  • Establishes and maintains incident reporting policy and processes for the department/organization in compliance with OPWDD regulations.
  • Report, follow-up, and close all incidents in accordance with OPWDD regulations.
  • Ensure notification to all required individuals in a timely manner. 
  • Ensure onboarding, annual, and as needed training for all required staff and program participants (e.g., family care providers, day habilitation, and comm hab staff)
  • Provide input and guidance to ensure investigations are timely, thorough, and contains complete and appropriate information.
  • Adheres to Incident reporting and coordination in accordance with OPWDD rules and regulations governing operation and conduct for Incident reporting. 
  • Cooperates and works professionally with others to ensure investigations and incident processes are conducted effectively and efficiently. 
  • Track and document all compliance issues and develop and implement mitigation strategies to reduce risk. 
  1. Communicates professionally and in a cooperative manner with coworkers and leadership.
  2. Keeps leadership informed of problems and presents them in a prompt and professional manner.
  3. Perform all other duties as assigned by the department leader. 
  4. Attend/support Urban League of Rochester events and fundraising functions, specifically ULR Membership Drive and United Way Campaign.

Requirements

QUALIFICATIONS:

  • Associates Degree preferred with relevant experience working with the developmentally disabled population or human services field
  • Must be proficient with Microsoft Office: Word, Excel, Publisher, Power-Point
  • Must have experience with Zoom, WebEx, Adobe Connect, or Microsoft Team 
  • At least two years’ experience in working with families and youth in an urban setting.
  • Minimum of one-year experience with developmentally disabled population.
  • Must be able to tolerate and handle stressful situations in a clam and professional manner.
  • Must be consumer service oriented.
  • Ability to relate well with others, a team player.
  • Valid NYS driver license, clean driver’s abstract.
  • Car necessary in some circumstances.
  • An equivalent combination of education and experience may be acceptable. 
  • OPWDD training preferred but will be offered
  • Must have Food Safety or Serve Safe Certification (or obtained within 60-Days of hire)
