Urban Health Plan is Hiring a Tracking Associate (Bilingual Spanish/English) - Recalls Near Bronx, NY
Position SummaryTracking Associates contact Referral Specialists to obtain consult reports and work closely to facilitate rescheduling appointments when necessary. Through phone calls, letters, and continual communication, the Tracking Associate is an important linkage between the patient and the referral provider(s).
This position is based out of the Bronx and requires a full-time commitment from Monday to Friday 8:00 am to 5:00 pm. The role is currently hybrid (work from home 4 days and working onsite 1 day), however candidates must be open to commuting to the Bronx onsite in the future.

Minimum Qualifications
- High School Diploma required, Associate Degree a plus.
- Administrative Medical Assistant highly desired.
- 1-2 years of administrative experience is required within healthcare setting.
- Bilingual Spanish/English required.
- Must be skilled in using Microsoft Word and Excel.
- Customer service skills are required.
- Ability to multi-task and effectively communicate with others.
- Assist in the tracking and recalling of patients and results.
- Follows-up referral sources.
- Prepare monthly reports.
- Conduct follow up and send letters to patients for reminders for preventive and chronic care services due.
- Perform hospital and ER tracking.
- Perform cancer screening tracking.
- Other duties as assigned by manager.
