Description

This position is flexible and part time approximately 15-20 hours per week. No evenings, weekends or holidays. 

Provide physical therapy assessments, reassessments, screens and interventions to PACE participants to promote and maximize physical and functional mobility.

• Conduct initial physical therapy evaluations, reassessments, and screens of PACE participants in accordance with PACE guidelines
• Educate and supervise staff, participants and family members regarding physical therapy rehabilitation principles
• Conduct home safety and community re-entry assessments
• Order durable medical equipment and recommend environmental modifications to maximize functional mobility within the home
• Provide adequate documentation including but not limited to safety evaluations, assessments, screens, treatment plans, progress notes in accordance with PACE guidelines
• Participate in daily and weekly clinical team meetings
• Foster participant confidentiality
• Contribute to quality improvement committees and program development projects
• Delegate clinical responsibilities appropriately
• Participate in rehabilitation meetings
• Delegate and implement appropriate treatment plans for Physical Therapy Assistants and Restorative Aides
• Actively seek opportunities for professional growth as a physical therapist in the adult and geriatric population
• Performs duties as requested by Supervisor
