Job Summary and Scope
Provide clinical pharmacotherapy, clinical pharmacology, and pharmaceutical services to patients receiving care within their area. Serve as a resource to primary providers. Develop and participate in programs and activities to ensure optimal use of medication therapies. Ensures the safe and effective use of medication by providing medication refill and medication therapy management services to optimize drug product selection. Perform Staff Pharmacist-Inpatient functions as assigned. Ensure adherence to University of New Mexico-Sandoval Regional Medical Center (UNM-SRMC) and departmental policies and procedures and compliance with State and Federal laws and regulations. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups. The pharmacist will display a commitment to the hospital mission statement and role model the SRMC Values and consistently demonstrate in thought and action the SRMC commitment to putting you first.
Essential Functions:
POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs
PROFESSIONAL DEVELOPMENT
Under Hospitals Bi-Laws and department protocol provide direct patient care by writing and renewing prescriptions
Develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols as outlined by state statute, UNM SRMC and Pharmacy Department policy and procedure
Perform non-invasive diagnostic and therapeutic procedures
Ensure the delivery of optimal written and verbal pharmaceutical care to patients with in their area of specialty
Evaluate and revise intervention care/treatment plan with patient, family and supervising provider as necessary to achieve improved health
Participate in the development of clinical protocols or order sets to ensure the appropriate and safe delivery of medications to patients; develop and provide information resources for patients to ensure the optimal adherence to medications prescribed
Continually evaluate current activities and propose new areas of involvement or changes in current practices
Provide accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicate them to other members of the health care team; maintain accurate, legible and confidential records
Develop and participate in programs to improve the quality of patient care and ensure the safe delivery of medications
Ensure departmental and hospital compliance with national quality standards appropriate to their area of practice (e.g., Joint Commission, CMS) through documentation of Quality Assurance
Develop and participate in a planned periodic chart review with supervising physician(s) that meets the requirements of the New Mexico Board of Pharmacy
Report and evaluate medication errors and adverse drug events and recommend systems for improving the safety of pharmaceutical care
Develop and provide educational programs for Pharmacists, other Healthcare Professionals, pharmacy technicians, interns, and/or Patients in the Community
Develop training and competencies and train SRMC Pharmacists and Technicians; other SRMC Healthcare professionals; Pharmacy Technicians; Pharmacy Interns; and patients
Work with other departments and healthcare professionals to develop programs to optimize the delivery of pharmaceutical care at SRMC
Actively work with other departments and healthcare professionals to optimize the delivery of pharmaceutical care; attends and participate in Department, Hospital, Medical Staff or Health Sciences Center committees as assigned
Design, implement, and report on projects that evaluate or improve the delivery of pharmaceutical care to patients including: Medication Use Evaluations; Formulary reviews; Therapeutic Class Reviews; Medication Safety Projects; Quality Improvement Projects; Performance Improvement Projects; and Cost-Reduction Projects
Follow patient safety-related policies, procedures and protocols
Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops.
Performs comprehensive medication reviews and addresses therapy adherence issues with patients and appropriately adjusts medications based on clinical findings.
Actively identify, mitigate, prevent, and report potential and actual drug-related problems or adverse drug events. Recommend rational therapeutic alternatives as needed to prevent future problems.
Interprets, evaluates and accurately processes medication requests in a timely fashion. Prior to authorizing prescriptions; screens for drug allergies, interactions, and appropriateness of medication, dose, frequency, and route of administration.
Authorizes refills for medications according to UNM Sandoval Regional Medical Centers policies, procedures, guidelines.

Maintain accurate records of encounters. Responsible for meeting all state, federal and regulatory requirements associated with the Pharmacist Clinician and the Ambulatory Care Service.
Collects data and supervises inspections for quality assurance and safety purposes. Maintains all logs and necessary paperwork associated with regulatory and quality assurance requirements. Actively participates in LEAN processes and quality/safety improvement initiatives.
Performs duties as an inpatient pharmacists when needed
Perform miscellaneous job-related duties as assigned by the Director of Pharmacy

Required Qualifications
Licensed Pharmacist in State of New Mexico or obtain within 90 days
Licensed Pharmacist Clinician in NM or obtain within 180 days
BLS certification within 90 days of hire
ACLS certification within 180 days of hire
PALS certification within 180 days of hire
Education/Experience
Education: BS in Pharmacy or PharmD
Preferred Educational/Experience Requirements:
PharmD degree
Residency in an acute care setting or
Residency in ambulatory care
Bilingual English/Spanish
Bilingual English/Keres,Tewa,Tiwa,Towa, Zuni, or Navajo
1 year directly related experience
Preferred Licensure/Certification:
Board Certification in Pharmacotherapy or ambulatory care
Supervision
Will report to Director of Pharmacy
