Company DescriptionOur mission is to provide quality community-based services for children and families affected by significant life changes, general mental health and/or developmental delays. In working with children and families,Unlocked Potential's staff utilize an individualized, strengths based, and holistically informed approach.Job DescriptionOur Clinicians offer “in home” counseling and therapeutic services to help support I/DD youths between the ages of 5 and 21.Common impacts to functioning may be due to a variety of reasons such as major life stress, trauma, or lifelong intellectual and developmental delays.As a scheduling consideration, families typically require youth services in the after school hours (3pm-8pm) and on weekends.Experience working with individuals diagnosed with Autism, Down Syndrome, Cerebral Palsy, or other intellectual/developmental delays.Locations:Middlesex County, NJPassaic County, NJBergen County, NJMonmouth County, NJEssex County, NJSomerset County, NJUnion County, NJQualificationsPunctually and consistently meet with each assigned child and family each week for the authorized number of hours (typically 2-4 hours per youth) to implement treatment plans which foster adaptive and social skills for the youth while educating and supporting the family around the youth’s needs.Keep and maintain accurate billing and treatment records.Work cooperatively with other identified team members to develop and implement a service plan on the youth’s behalf.Must be willing to go into the home of the families served for the authorized number of hours per week based on each referral.Must possess a valid NJ Clinical License (LAC or LSW ).Must possess a driver’s license and access to reliable transportation (vehicle must be documented by the agency to be insured and registered).Must consent to and pass a criminal background check.Must have basic computer and cell phone knowledge and be able to comply with all HIPAA (confidentiality) laws as it relates to Protected Health Information.Must be able to continuously complete role functions requiring sufficient physical ability, mobility, vision, hearing, and communication when providing direct services to youth and families in the home, community, and/or via telehealth.Must be reliable, punctual, organized, and a self-starter.Must be creative, enthusiastic, and a team player!Bilingual or sign language a plus.Additional InformationAdditional Perks:Paid Time offPaid for required meetings/trainingsEmployee discounts through our discount program!!Employee assistance program!!Unlocked Potential is an Equal Opportunity-Affirmative Action Employer – Minority / Female / Disability / Veteran / Gender Identity / Sexual Orientation / Age.Unlocked Potential, LLC is a community-based behavioral health care provider that began providing services in January 2015.
